Application Dossier for Re-issuance of Securities Practitioner Certificate

Based on Clause 2, Article 214 of Decree 155/2020/ND-CP stipulates as follows:

- Dossier for reissuing a securities practicing certificate:

+ In cases where the securities practicing certificate has been revoked under the provisions of points a and c, Clause 3, Article 97 of the Securities Law, the dossier for reissuing the securities practicing certificate shall include the documents stipulated in Clause 4, Article 213 of this Decree;

+ In cases where the securities practicing certificate is lost, damaged, or where the identity verification information in the securities practicing certificate is changed, the dossier for reissuing the securities practicing certificate shall include the following: An application for reissuing the securities practicing certificate according to Form No. 85 of the Appendix issued together with this Decree; a valid identity card or citizen identity card or passport; 02 photos of 4cm x 6cm taken within 06 months up to the date the State Securities Commission receives the dossier; the securities practicing certificate previously issued by the State Securities Commission, except in cases of loss of the certificate.
