Pursuant to the Law on Local Government Organization dated June 19, 2015;
Pursuant to the Law amending and supplementing a number of articles of the Law on Organization of the Government of Vietnam and the Law on Local Government Organization dated November 22, 2019;

Notes on abbreviations:
- N: Poor Household; CN: Nearly Poor Household;
- Numbers from 1 - 12 are abbreviations for indicators of basic social service shortages for poor and nearly poor households:
1. Employment; 2: Dependents in the household; 3: Nutrition; 4: Health insurance; 5: Level of adult education; 6: School attendance status of children; 7: Housing quality; 8: Average housing area per person; 9: Drinking water source; 10: Sanitary latrine; 11: Use of telecommunications services; 12: Means to access information.
(Mark X in the corresponding box with the classification result of the poor household, nearly poor household, and the shortages of the poor household, nearly poor household)
IMPORTANT NOTES
1. The Certificate of poor household/near-poor household is issued to the household as a basis to determine the household and its members eligible for the policies supporting poor households, nearly poor households.
2. According to the Decision recognizing the annual list of poor households, nearly poor households in the area, the Chairman of the People's Committee of the commune signs, seals the certification of poor household/near-poor household for the household in the column of the year immediately following the review time, as a basis for citizens to benefit from policies supporting poor households, nearly poor households in the year at the signature, seal column.
3. In case the household has changes in members, relevant documents regarding the changes (marriage certificate, birth certificate, household registry transfer, death certificate, etc.) should be sent for the Chairman of the People's Committee of the commune to consider, add to the list of household members in the Certificate of poor household/near-poor household. The Chairman of the People's Committee of the commune signs and seals the row recording the changed member's name in the Notes column in the List of Household Members table.
4. The Certificate of poor household/near-poor household must be carefully preserved to avoid damage, tearing, or loss. In case it is damaged, torn, or lost, the Chairman of the People's Committee of the commune considers and issues a replacement for the household based on the list and data of poor households, nearly poor households in the locality managed by the People's Committee of the commune and retrieves the old Certificate of poor household/near-poor household for file keeping./.
