Application Dossier and Procedures for Supplementing or Reissuing Certificates for Persons with Meritorious Services and Relatives of Martyrs
Pursuant to Article 115 of Decree 131/2021/ND-CP (Effective from February 15, 2022) stipulating the application dossier and procedures to supplement or reissue certificates for persons with meritorious services and relatives of martyrs as follows:
- Individuals shall submit an application according to Form No. 25 of Appendix I of this Decree to:
+ The agency or unit directly managing those with meritorious services currently serving in the military or police.
+ The Commune-level People's Committee where the persons with meritorious services reside, managed by the Labor - Invalids and Social Affairs sector or the relatives of martyrs.
- The direct managing agency or the Commune-level People's Committee has the responsibility to verify the application within 5 working days, compile a list and send the application to the dossier management agency.
- The dossier management agency, within 12 days from the date of receiving all documents, shall review and issue a supplementary or reissued certificate.
- The Ministry of National Defence and Ministry of Public Security provide guidelines for the process of supplementing or reissuing certificates for persons with meritorious services under their management. The review and processing time shall not exceed 20 days from the date of receiving the application.
- Certificates for persons with meritorious services or relatives of martyrs issued before July 1, 2021, remain valid. Reissuance shall follow Form No. 102 of Appendix I of this Decree.
Application Dossier and Procedures for Issuing Certificates of Relatives of Persons with Meritorious Services
According to Article 116 of Decree 131/2021/ND-CP (Effective from February 15, 2022) stipulating the application dossier and procedures to issue certificates of relatives of persons with meritorious services as follows:
- Persons with meritorious services or their relatives shall submit an application for issuance of a certificate of relatives of persons with meritorious services to the Commune-level People's Committee where they reside, accompanied by a certified copy of one of the following documents:
+ Certificate of meritorious services or Commemorative Medal for those participating in revolutionary, resistance, protecting the Nation, fulfilling international obligations, who were imprisoned by the enemy; Resistance Medal; Victory Medal; Commemorative Medal "The Nation Remembers"; Certificate "Having Meritorious Services to the Nation."
+ Documents issued by competent authorities, certifying the relationship with the person with meritorious services including: citizen identification card or household registration book issued before July 1, 2021.
- The Commune-level People's Committee where the persons with meritorious services reside has the responsibility to issue the certificate of relatives of persons with meritorious services within 5 working days from the date of receiving all documents, according to Form No. 45 of Appendix I of this Decree.
