The Identification Information Receipt Form is created by the authority responsible for managing identification cards at the place where the application for issuance, replacement, or reissuance of identification cards or identification certificate is received. It is based on the extracted information of citizens, Vietnamese nationals of undetermined nationality residing in Vietnam, from the National Database on Population and Identification Card Database during the procedure.
[2] Appointment Letter for Results of Identification Procedure (referred to as CC02)
The Appointment Letter for Results of Identification Procedure is issued by the authority responsible for managing identification cards at the place where the application is received. It is given to citizens, Vietnamese nationals of undetermined nationality, to schedule a date for returning the results of the identification procedure.
[3] Notice of Rejection of Identification Procedure (referred to as CC03)
The Notice of Rejection of Identification Procedure is created by the authority responsible for managing identification cards at the place where the application is received. It is used to inform citizens, Vietnamese nationals of undetermined nationality, about the rejection of their identification procedure.

1. Population Information Collection Form (referred to as DC01)
The Population Information Collection Form is used for citizens, Vietnamese nationals of undetermined nationality to provide information for collection and updating in the National Database on Population.
2. Request Form for Identification Procedure (referred to as DC02)
The Request Form for Identification Procedure is used when citizens, Vietnamese nationals of undetermined nationality need to go through the processes of issuance, replacement, or reissuance of identification cards, confirming the information of the 9-digit personal identification number, canceling or reestablishing the personal identification number, extracting information from the National Database on Population, extracting information from the Identification Card Database, adjusting information in the National Database on Population, integrating information into the identification card, collecting and updating information in the Identification Card Database.
3. Information Provision Form in the National Database on Population and Identification Card Database (referred to as DC03)
The Information Provision Form in the National Database on Population and Identification Card Database is issued by the authority responsible for managing identification cards or by the local police at the commune level. It is used to provide information from the National Database on Population and Identification Card Database to agencies, organizations, and individuals requesting information extraction from the National Database on Population and Identification Card Database.

What are responsibilities of the provincial police in management of identification cards in Vietnam?
Based on Article 13 of Circular 17/2024/TT-BCA, the provincial police are responsible for the management of identification cards as follows:
- Directly implement and advise the Provincial People's Committee on the organization of the implementation of legal documents regarding the National Database on Population and Identification Card Database, issuance, and management of identification cards.
- Organize the collection, updating, standardization, digitization, and information extraction from the National Database on Population and Identification Card Database. Organize the reception and management of applications for identification cards and certification documents.
- Conduct inspections, audits, handle complaints, denunciations, and address violations related to the collection, updating, adjustment, extraction, and use of information from the National Database on Population and Identification Card Database, as well as the issuance and management of identification cards within their jurisdiction.
- Regularly and promptly report on the issuance and management of identification cards in the area of their jurisdiction to the Ministry of Public Security.
