Procedure for Re-issuance of Health Insurance Cards for Children under 6 Years Old

According to Clause 4, Article 27 of the issuance process attached to Decision 595/QD-BHXH in 2017, there are provisions on the dossier for reissuance, change of health insurance cards as follows:

Reissuing and changing health insurance cards

4.1. Dossier components

a) Participants

- Declaration of participation, adjustment of information of Social Insurance, Health Insurance (Form TK1-TS).

b) Unit: Information list (Form D01-TS).

4.2. Number of dossiers: 01 set.

And in Article 30, the timeframe for the reissuance is specified as follows:

- In case there is no information change: on the same day upon receiving the complete dossier as prescribed.

- In case of information change: no more than 03 days from the date of receiving the complete dossier as prescribed.

- In case the participant is undergoing treatment at medical facilities, it will be completed on the same day upon receiving the complete dossier as prescribed.

=> Thus, according to the above-mentioned regulations, the dossier and procedures for reissuing health insurance cards and the timeframe for reissuing health insurance cards for children under 6 years old are implemented as stated above.
