Is there any document that specifies method to declare the TK1-TS declaration form for participation, adjustment of social insurance information, health insurance? Please guide me on method to declare the information in this form.

INSTRUCTIONS FOR FILLING OUT
TK1-TS DECLARATION FORM: Declaration form for participation, adjustment of social insurance and health insurance information
[01]. Full name: write the full last name, middle name, and first name in capital letters with accents of the person participating in social insurance, health insurance, labor accident insurance, and occupational disease insurance.
[02]. Date of birth: write the full date of birth as stated in the birth certificate or citizen identity card, passport, or ID card.
[03]. Gender: write the gender of the participant (if male, write "nam" or if female, write "nu").
[04]. Nationality: write as stated in the birth certificate or citizen identity card, passport, or ID card.
[05]. Ethnicity: write as stated in the birth certificate or citizen identity card, passport, or ID card.
[06]. Place of birth registration: clearly write the name of the commune (ward, town); district (urban district, town, city belonging to the province), province, city where the birth certificate was registered.
In case the place of first birth registration cannot be determined, write the place of origin (in case of merger, division of administrative boundaries, write according to the place name at the time of declaration) or write according to the place of permanent or temporary residence registration.
[07]. Mailing address: write the complete address of the current place of residence for the social insurance agency to send back the social insurance book, health insurance card, or the results of other administrative procedures: house number, street, hamlet; commune (ward, town); district (urban district, town, city belonging to the province); province (city).
[08]. Full name of father or mother, or guardian (applicable to children under 6 years old): Write the full name of the father or mother or guardian.
[09]. Social insurance number: write the social insurance number issued by the social insurance agency (only applicable to cases where the social insurance number has been issued when adjusting information); if the social insurance number is not remembered, the declarant can look up the social insurance number at the Cultural Post Office or at the address: http://baohiemxahoi.gov.vn or coordinate with the social insurance agency or the labor user unit, commune People's Committee, collection agency/nursing school, social assistance facility, nursing and rehabilitation facility for wounded soldiers and people with meritorious services; vocational education institution under the Ministry of Labor, War Invalids and Social Affairs and the management agency of the subject to determine the social insurance number.
In case the social insurance number cannot be determined but there is a social insurance book or health insurance card number, write the social insurance book number or health insurance card number.
In case the social insurance number is determined but different from the social insurance book number, write the social insurance number in this item and write the additional social insurance book number in item [14].
[09.1]. Contact phone number: write the contact phone number (if any).
[09.2]. Citizen identity card/Passport/ID card number: write the identification card number or write the passport number or write the ID card number.
[10]. Family code: write the family code issued by the social insurance agency (only applicable to cases where the social insurance number has been issued when adjusting information); if the social insurance number is not remembered, the declarant can look up the social insurance number at the address: http://baohiemxahoi.gov.vn or coordinate with the social insurance agency or the labor user unit, commune People's Committee, collection agency/nursing school, social assistance facility, nursing and rehabilitation facility for wounded soldiers and people with meritorious services; vocational education institution under the Ministry of Labor, War Invalids and Social Affairs and the management agency of the subject to determine the family code.
[11]. Contribution amount (applicable to voluntary social insurance participants): write the monthly income chosen by the voluntary social insurance participant.
[12]. Contribution method (applicable to foreign workers, voluntary social insurance participants): specify the contribution method as 03 months or 06 months or 12 months ...
[13]. Initial health insurance registration place: write the place of initial registration for health examination and treatment under health insurance (the list of places of initial health examination and treatment registration is announced annually by the social insurance agency and sent to the unit, commune People's Committee, collection agency).
[14]. Change request content: write the requested change content such as: full name, date of birth, related information about title, occupation, job, contribution method, initial health insurance registration place ...
[15]. Attached documents:
- For information adjustment participants, write the types of supporting documents.
- For participants entitled to higher health insurance benefits, write the types of supporting documents.
After completing the declaration, the participant must sign and write their full name. In case there are changes in personal information (last name, middle name, first name; date, month, year of birth, gender) already written on the social insurance book, health insurance card, confirmation from the unit where the worker is currently employed is required. For those who are preserving the social insurance contribution period, confirmation is not required.
Appendix: Household members
* General information section: write the full name of the head of household; write the household registration book number (or temporary residence book number); contact phone number (if any); clearly write the address: hamlet (village, neighborhood); commune (ward, town); district (urban district, town, city belonging to the province); province (city).
In case the household only has a temporary residence certificate, the declaration is still required but write the phrase "temporary residence certificate" in the note column.
* Criteria by column:
- Column A: write the serial number from 1 to all members in the household.
- Column B: write the full name of each person in the household registration book (excluding temporarily absent persons) or temporary residence book (including the head of household and household members).
- Column 1: write the social insurance code for each household member issued by the social insurance agency; in cases where the social insurance code is not determined, the declarant can look up the social insurance code at the address: http://baohiemxahoi.gov.vn (if eligible).
- Column 2: write the date, month, year of birth as in the birth certificate or ID card, passport, citizen identity card.
- Column 3: write the gender of the household members (if male, write "nam" or if female, write "nu").
- Column 4: write the name of the commune (ward, town); district (district, town, city under the province); province, city where the birth certificate was issued.
In cases where the place of birth registration is not determined, write the native place (in cases of administrative boundary merging, splitting, write according to the place name at the time of declaration) or write according to the place of permanent or temporary residence registration.
- Column 5: write the relationship with the head of household (wife, husband, child, grandchild...).
- Column 6: Identity card/Passport/Citizen identification card number (if available): write the identity card number or passport number or citizen identification card number.
- Column 7: write any necessary notes.
After completing the declaration, the declarant signs and writes their full name.
