Incorrect discharge date: Can a new discharge certificate be requested?
According to Clause 5, Article 26 of Circular 56/2017/TT-BYT stipulating the responsibilities of medical examination and treatment facilities as follows:
Medical examination and treatment facilities that have issued hospital discharge certificates, birth certificates, pregnancy leave certificates, certificates of incapacity to care for the child after birth, and social insurance leave certificates have the responsibility to:
Reissue hospital discharge certificates, birth certificates, pregnancy leave certificates, certificates of incapacity to care for the child after birth, and social insurance leave certificates in the following cases:
- Lost or damaged;
- The person who signed the certificates was not authorized;
- The stamps on the certificates are not conformable;
- There are errors in the information recorded on the hospital discharge certificates, birth certificates, pregnancy leave certificates, certificates of incapacity to care for the child after birth, social insurance leave certificates.
In the case of reissuing, a "Reissued" stamp must be affixed on the hospital discharge certificates, birth certificates, pregnancy leave certificates, certificates of incapacity to care for the child after birth, and social insurance leave certificates.
Therefore, in case there are errors in the information recorded on the hospital discharge certificate, including the discharge date, if the discharge date on your hospital discharge certificate is incorrectly issued by the medical facility, you can return to the issuing facility to complete the reissuance procedure.
