How to fill out the Social Insurance, Health Insurance, and Unemployment Insurance registration form (form TK1-TS)?

You can refer to the guidance on how to fill out the social insurance, health insurance, and unemployment insurance registration form (form TK1-TS) as follows:

Section I: Applied to participants who do not find their social insurance number by searching, must fill from criteria [1] to criteria [11] (except criterion [8]) to participate in social insurance, health insurance as follows:

[01]. Full Name: enter the participant's full name in capital letters with diacritical marks.

[02]. Gender: enter the participant's gender (if male, write "male" or if female, write "female").

[03]. Date of Birth: enter the participant's full date of birth as in the birth certificate or identity card/citizen identification/passport.

[04]. Nationality: enter the participant's nationality as in the birth certificate or identity card/citizen identification/passport.

[05]. Ethnicity: enter the participant's ethnicity as in the birth certificate or identity card/citizen identification/passport.

[06]. Citizen Identification/Personal Identification/Passport Number: enter the citizen identification/personal identification or passport number of the participant issued by the competent authority (including children under 6 years old with a personal identification number).

[07]. Phone Number: enter the participant's phone number or a contact phone number for the participant.

[08]. Email: enter the participant's email address or a contact email address for the participant (if any).

[09]. Birthplace Registration: specify the name of the commune; district; province where the participant was registered for birth. In case the birthplace cannot be determined, enter the native place or the permanent or temporary residence address (following the 3 levels similar to the birth registration place).

[10]. Parent/Guardian's Name (for children under 6 years old): enter the name of the parent or guardian for children under 6 years old.

[11]. Result Delivery Address: choose the method of receiving administrative procedure results.

For receiving the social insurance book, health insurance card in paper form; specify the full residential address (house number, street/village; commune; district; province) so the social insurance agency can send the documents, social insurance book, health insurance card, or other administrative procedure results.

[12]. Declare the Household Member Appendix (attached appendix) for participants who do not find their social insurance number and for household-based health insurance participants to reduce the payment according to the guidance.

After completing the declaration:

+ The participant should note the content: has searched but did not find the social insurance number, voluntarily declares, provides relevant documents to ensure accurate information, and takes responsibility before the law for the declared content; sign and clearly state the full name.

+ The employer (participants preserving the social insurance contribution period do not need to confirm) should note the content: confirm the adjusted information of the participant is correct with the management dossier and takes responsibility before the law for the confirmed content; sign, stamp, and clearly state the full name.

Employee Rights when Participating in Social Insurance

Article 18 of the Social Insurance Law 2014 stipulates the benefits when participating in social insurance as follows:

- Participate in and enjoy social insurance benefits as stipulated by the Social Insurance Law 2014.

- Issuance and management of the social insurance book.

- Receive pension and social insurance benefits fully and promptly, through one of the following payment methods:

+ Directly from the social insurance agency or authorized service organization.

+ Through the employee's bank account.

+ Through the employer.

- Health insurance benefits in the following cases:

+ Receiving pension.

+ On maternity leave for childbirth or adopting a child.

+ On labor accident or occupational disease monthly benefits.

+ On sickness benefits for long-term treatment as per the Ministry of Health's regulation.

- Proactively undergo medical examination to assess the reduction in work capacity if falling under the provisions of Point b, Clause 1, Article 45 of the Social Insurance Law 2014, and preserving the social insurance contribution period.

- Reimbursement of medical examination fees if eligible for social insurance benefits.

- Delegating others to receive pension, social insurance benefits.

- Periodically every 06 months, the employer provides information about social insurance contributions.

- Annually, the social insurance agency confirms social insurance contributions.

- Requesting the employer and the social insurance agency to provide information on social insurance contributions and benefits.

- Complaints, denunciations, and lawsuits on social insurance as prescribed by law.
