Health Assessment Dossier for Early Retirement

According to Clause 3, Article 5 of Circular 56/2017/TT-BYT, the medical examination dossier for pension implementation for employees includes the following documents:

- An introduction letter from the employer in the form prescribed in Appendix 1 attached to Circular 56/2017/TT-BYT for employees currently enrolled in mandatory social insurance;

- The original or lawful copies of one or more of the following medical examination and treatment documents: summary of the medical record, disability certificate, hospital discharge certificate, medical examination book, copies of occupational disease dossier, the expert appraisal record of occupational disease, labor accident for individuals who have been medically examined for occupational disease, or labor accident;

- One of the following photo-identification documents: Identity card; Citizen identification; Valid passport. In case these documents are not available, a Confirmation Certificate from the local police with a photo affixed, stamped across the photo, and issued within 3 months from the proposal for the medical examination is required.
