Application for issuance of certificate of eligibility for HIV testing in Vietnam

Decree No. 75/2016/NĐ-CP was issued by the Government of Vietnam, providing conditions for HIV testing.

According to Decree No. 75/2016/NĐ-CP of Vietnam’s Government, the applicant shall submit 01 application to a competent authority, including:

- An application form;

- A list of person in charge and testers of the applicant;

- A list of equipment of the applicant;

- A site plan of the testing area;

- Copies of confirmations of HIV testing results.

Also according to Decree No. 75/2016/NĐ-CP, an application for reissuance of a certificate of eligibility consists of the application form No. 02 enclosed herewith.
