Guidelines for specific treatment of strongyloidiasis in Vietnam in 2022?

In sub-section 5.2, Section 5, Guidance issued together with Decision 1384 / QD-BYT of 2022 guiding the diagnosis, treatment and prevention of strongyloidiasis, issued by the Minister of Health, regulates the specific treatment of helminthiasis. Intestinal eels are as follows:

Use one of the following regimens

Regimen 1: Ivermectin 3mg, 6mg tablets

(1) Dosage

+ Children> 2 years old: 400mg / time / day for 3 consecutive days.

+ Children <2 years old: 200mg / time / day for 3 consecutive days, can be repeated after 3 weeks.

- The form of serious disease, diffuse strongyloidiasis has complications using a dose of 400mg / time X 2 times / day for 7 days. Follow-up test of strongyloid larvae in 2-4 weeks, if the larvae remain, the dose is continued as before.

(2) Contraindications

- Having a history of hypersensitivity to the compounds of the benzimidazol type or to certain ingredients of the drug.

- Patients with a history of bone marrow toxicity.

- Women who are pregnant in the first 3 months or are breastfeeding.

- Children under 2 years old.

Regimen 3: Thiabendazol 500 mg tablet

(1) Dosage: 25mg / kg / time X 2 times / day (maximum 3g / day). drink after eating.

- For the normal form: 2-day treatment.

- For diffuse strongyloidiasis: treatment lasts a minimum of 5-7 days or until the larvae are cleared.

(2) Contraindications

- In cases of hypersensitivity to any ingredient of the drug.

- Pregnant and lactating women.

- Pediatric patients weighing less than 13.6 kg.

How to treat symptoms of strongyloidiasis in Vietnam?

- Monitoring of inpatient treatment:

+ The patient is monitored at an inpatient facility for about 7 days.

+ Retest stool in 3 days on Thursday, 6th, 7th during inpatient treatment, blood count test, biochemistry of liver and kidney function to evaluate organ function of the body.

+ For patients with underlying diseases, do tests to monitor and treat related underlying diseases.

- Follow-up after inpatient treatment:

The indicators evaluate the effectiveness of treatment, after the treatment period of 1 month, 3 months, 6 months and 12 months.

+ Clinical: clinical symptoms decrease or disappear

+ Test the stool or duodenal fluid or sputum without strongyloid larvae

+ Complete blood count: the number of eosinophils returned to normal or decreased

+ Biochemistry: liver, kidney function

+ Strongyloid ELISA, IgE reevaluated after 3 months, 6 months, 12 months and longer if still positive.

+ Ultrasound of the abdomen: the loops of the small intestine return to normal.

+ CT scan, MRI of the brain: the damage is reduced or gone.
