How is the organization of pre-vaccination screening carried out at the hospital according to regulations?
Pursuant to subsection 2, Section IV, Guidance for pre-vaccination screening for children issued together with Decision No. 1575/QD-BYT in 2023, the organization of pre-vaccination screening at hospitals must ensure specifically include the following:
(1) Executor
- Pre-vaccination screening, examination, evaluation, coordination with specialist doctors (if necessary), comments and conclusions on indications for vaccination for children.
- Specialist doctor: examine and evaluate the child's medical condition according to the specialty; coordinate with the vaccination consultant to draw conclusions about the indications for vaccination for children.
- Health workers involved in counseling, vaccination and monitoring of post-vaccination reactions.
(2) Equipment
- Having enough facilities to carry out screening and specialized examinations.
- Pre-vaccination screening sheet for subjects aged 1 month and older (Appendix III issued together with Decision No. 1575/QD-BYT in 2023) and pre-vaccination screening test card for newborns (Appendix IV issued together with Decision No. 1575/QD-BYT in 2023).
(3) Steps to perform pre-vaccination screening
- Doctor consulting, screening for vaccination: comprehensive assessment of immunization conditions for children; consult a specialist doctor about the medical condition according to the specialty (if necessary). Make conclusions about vaccination conditions for children.
- Specialist doctor: when required, fully assess the medical condition according to the specialty to be consulted; make comments and suggestions related to vaccination about the child's medical condition. Coordinate with vaccination staff at the hospital to vaccinate and follow up after vaccination for children when needed.
(4) Record screening and keep records
- To record and keep the vaccination database in accordance with current regulations and regulations on recording medical examination books and medical records for children who come for outpatient and inpatient examination and treatment at the hospital.
- In case there is a medical record at the hospital: all contents of the screening examination in the checklist and the order to prescribe the injection must be recorded in the medical record or the pre-vaccination screening examination sheet is kept with the medical record as prescribed.
- In case there is no medical record at the hospital: The pre-vaccination screening examination sheet is stored at the vaccination site and on the National Immunization Information Management System. Save checklist for 15 days.
(5) Assessment of health status and vaccinations for cases sent from lower levels and pediatric patients before discharge:
- Receive and organize screening, appointment and vaccination for cases sent from lower levels and pediatric patients being treated at the facility before being discharged from the hospital.
- For cases of children with congenital immunodeficiency or confirmed HIV infection, it is necessary to write in full medical examination information, immune status along with immunological test results (if any) and indications/antibiotics. Indication/delay for live, attenuated vaccines.
- Notify the vaccination facility outside the hospital, advise and guide parents/caregivers about the next vaccinations.
