Procedures for issuance of the practicing card for treatment of plant quarantine in Vietnam

According to Article 8 of Circular No. 05/2015/BNNPTNT of the Ministry of Agriculture and Rural Development of Vietnam, sequence and procedures for issuance of the practicing card for treatment of plant quarantine subjects are specified as follows:

Firstly, the application includes:

- Application form according to Form prescribed in Appendix IV enclosed herewith;

- Certificate of training in professional competence (copy);

- Health certificates (original) issued by medical agencies from commune level and over;

- Two 2 cm x 3 cm photos.

Persons who apply for issuance of the practicing card must submit one (01) set of application directly or by post to the Plant Protection Department.

The Plant Protection Department shall inspect eligibility of the application right after the application is directly received or within one working day for application sent by post.

Within 05 working days since receipt of the application, Plant Protection Department shall carry out issuance of the practicing card according to Form as prescribed in Appendix VI enclosed herewith, or otherwise issue a written notice specifying the reasons why the practicing card is not issued.

Besides, Circular No. 05/2015/BNNPTNT also stipulates that the practicing card shall be re-issued in case of being lost, damaged or having mistakes made. Application, sequence and procedures for re-issuance of the practicing card are prescribed in Article 8 hereof.
