Health Classification for Military Service
1. Basis of health classification
According to health standards specified in Table 1, Table 2, and Table 3 of Appendix 1 issued with this Circular.
2. Scoring method
Each criterion, after examination, the doctor will score from 1 to 6 in the "Score" column, specifically:
a) Score 1: Indicates very good health status;
b) Score 2: Indicates good health status;
c) Score 3: Indicates fairly good health status;
d) Score 4: Indicates average health status;
dd) Score 5: Indicates poor health status;
e) Score 6: Indicates very poor health status.
3. Guidance for filling out the pre-enlistment medical check-up note
a) Each specialty, after medical check-up, the doctor will score in the "Score" column; the "Reason" column must concisely state the reason for the score; in the "Sign" column, the examining doctor must sign and write their full name;
b) In the conclusion section, the President of the pre-enlistment medical check-up council will base on the scores given for each criterion to conclude, classify health status correspondingly, recorded numerically and in words (the part in words should be in parentheses);
c) The President of the pre-enlistment medical check-up council is responsible for signing the pre-enlistment medical check-up note after concluding;
d) The signature of the President of the pre-enlistment medical check-up council must be stamped with the seal of the President's agency; the signature of the President of the medical re-check-up council must be stamped with the seal of the unit that decided to establish the medical re-check-up council.
