The place receiving the application form will keep the cover containing the Application Form, form number 1, a photocopy of ID card/citizen ID card/passport, and 2 photos. Form number 2 will be returned to the candidate after it has been signed and stamped for confirmation.

How to fill out the application form for the 2024 Vietnam High School Graduation Exam?

(1) 12th grade students must complete the online registration form for the exam:

The Ministry of Education advises that before filling out the application form for registration, students must carefully read the sections and the INSTRUCTIONS FOR FILLING OUT THE FORM, If there are any unclear points, students should ask the registration staff for detailed guidance. Students are fully responsible for the accuracy of the information provided in the application form.

12th grade students must fill out the registration information online using the account provided on the Exam Management Software System. After completing the review and confirmation within the specified registration period, the high school where the student is studying will print out the application form, form number 1, form number 2, and sign and stamp them to confirm the identity of the student.

(2) Independent candidates fill out the necessary information on the application form cover:

Independent candidates are free to fill out the necessary information on the application form cover, form number 1, and form number 2 (the information must match in all corresponding sections on the cover, form number 1, and form number 2). They must then submit it to the registration office along with a photocopy of both sides of their ID card/citizen ID card/passport on an A4 sheet of paper and 2 photos sized 4x6, resembling the ID card/passport style, taken within the last 6 months (with full name, date of birth, and the date the photo was taken written on the back of each photo, and both photos placed in a small envelope).

In addition, one more photo must be attached in the designated position on the front of the envelope containing the application form. The local police at the place of residence of the independent candidate will sign and stamp the photo on the application form to confirm the candidate's identity.

(3) How to fill out the form for those who already have a high school diploma and register for university/college admission:

Those who do not have a high school diploma or have a vocational diploma and are taking the exam to qualify for a high school diploma or using the high school diploma exam results as a basis for university/college admission must fill out all sections on the software.

Those who already have a high school diploma or a vocational diploma and register for the exam to use the high school diploma exam results as a basis for university/college admission only need to fill out sections 1 to 15 and sections 24, 25, 26, 27 on the software.

The registration office will keep the application form cover, form number 1, a photocopy of the ID card/citizen ID card/passport, and 2 photos, and return form number 2 to the candidate after it has been signed and stamped for confirmation.

Candidates should keep form number 2 to receive the Exam Notice and Certificate of Exam Results. In case of any inaccuracies in the registration information or if the Exam Notice is lost, candidates should bring this application form directly to the registered Exam Center during the orientation session to request corrections and complete the exam procedures.

How to fill in the 27 sections on the 2024 High School Graduation Application Form:

- Section "Provincial Department of Education and Training"............. CODE: Candidates who register for the exam at the registered testing unit under which Provincial Department of Education and Training should write the name of the Provincial Department of Education and Training in the blank space, then fill in the 2 digits representing the code of the Provincial Department of Education and Training in the next 2 blank spaces, the code of the Provincial Department of Education and Training is regulated by the Ministry of Education and Training.

- Section "Receipt Number": The receiving location for registration does not need to be filled in by the candidate.

- Sections 1 and 2: Fill in according to the instructions on the application forms for the high school graduation exam.

- Section 3:

a) Place of birth of the candidate only needs to specify the name of the province or centrally-run city (province/city), if born abroad, the candidate only needs to specify the name of the country (in Vietnamese),

b) Ethnicity should be written correctly according to the birth certificate,

c) For foreign nationality, mark (X) in the box next to it.

- Section 4: The Citizen Identification Card Number, Identity Card Number, Personal Identification Number, and Passport Number are written together as the Citizen Identification Card/Identity Card number in this section. For the new model of Citizen Identification Card/Identity Card, write all 12 digits in the corresponding boxes; for the old model of Identity Card, write 9 digits in the last 9 boxes on the right, leave the first three boxes blank.

- Section 5: Province/city code, district code, and commune/ward code only apply to communes/wards belonging to Region 1, as regulated by the Ministry of Education and Training. Candidates need to look up the correct province/city code, district code, and commune/ward code at the place of registration to fill in the corresponding boxes on the right.

Candidates who do not have a current residence in a commune in Region 1 can leave the commune code box blank.

After filling in the administrative unit codes in the boxes, candidates should write the name of the commune/ward, district, province/city in the blank line. For candidates who are eligible for priority objects or regions related to their current residence, it is necessary to confirm the period of residence for over 18 months in Region 1 or over 18 months in particularly difficult communes, communes with particularly difficult hamlets during their high school study period by marking in the corresponding box.

- Section 6: Write the name of the school and the district, province/city address of the school on the dotted line. Write the province code where the school is located in the first 2 boxes, and write the school code in the next 3 boxes (the school code is written according to the regulations of the Department of Education and Training, if the school code has 1 digit, write 0 in the first two boxes, if the school code has 2 digits, write 0 in the first box). For candidates who are police officers or military personnel participating in the examination for admission to universities and colleges, write the corresponding province/city code for the province where they are stationed and the high school code is 900.

For candidates who have studied abroad, write the corresponding province/city code for the province/city of permanent residence in Vietnam and the high school code is 800. Class name: write "TDO".

- Section 7: Clearly write the phone number and email. For candidates who request to adjust their online admission registration, they need to register their mobile phone number to be provided with a one-time password (OTP) via text message to ensure security during online admission registration.

- Section 8: Candidates must clearly write the contact person's information: full name, phone number, address of the hamlet (house number), village (street, alley), commune/ward, district, province/city. This address will also be the address to receive the Admission Notice if the candidate is admitted.

- Section 9: Candidates who wish to receive the examination results for admission to universities and colleges in Early Childhood Education should mark (X) in the box next to it.

- Section 10: Candidates must mark (X) in one of the two boxes to indicate whether they study according to the high school program or the General Education Continuing Education Program at the high school level.

- Section 11: Candidates must mark (X) in one of the two boxes to indicate whether they are independent candidates who have not graduated from high school or have graduated from high school (as of the time of the examination).

- Section 12: Candidates who register to take the examination at a specific Examination Council must write the name of the Examination Council and the Examination Council code as regulated by the Ministry of Education and Training in the corresponding position.

- Section 13: Submit the application form at the locations specified by the Department of Education and Training. The registration unit code is written according to the instructions of the receiving location.

- Section 14: For current 12th grade students (who have not graduated from high school), they must register for the exam at point a, and they are not allowed to choose any component subjects at point b.

For independent candidates, depending on their examination purposes and their choice of university or college entrance exam subject combinations, they can choose to take the entire exam (at point a) or only select some component subjects (at point b) that are appropriate. Independent candidates can only register for one combination exam (either natural sciences or social sciences). Independent candidates are only allowed to register for component subjects within the same combination exam.

In the case of independent candidates who have not graduated from high school, if they have met the conditions for preserving previous exams/subjects (to fulfill high school graduation requirements), they can choose to preserve the results of those exams/subjects by entering their scores in Section 16. However, they can still choose to take the component exams/subjects (which were requested to be preserved) in order to obtain the results for university or college entrance.

For students following the Continuing Education program, they can choose the Foreign Language exam if they wish to use the Foreign Language subject in their university or college entrance exam subject combination.

How to choose component exams/subjects: Candidates mark (X) in the corresponding box of the desired component exam/subject. For the Foreign Language exam, candidates fill in the specific language code as follows: N1 - English; N2 - Russian; N3 - French; N4 - Chinese; N5 - German; N6 - Japanese; N7 - Korean. Candidates can only choose component subjects within one combination exam.

Mục 15: Đối với thí sinh có nguyện vọng miễn thi bài thi môn Ngoại ngữ, cần ghi rõ loại chứng chỉ đủ điều kiện miễn thi hoặc ghi rõ là thành viên đội tuyển quốc gia dự thi Olympic quốc tế môn Ngoại ngữ theo quy định của Bộ GDĐT đối với loại chứng chỉ có ghi điểm thi (điểm toàn bài thi), thí sinh phải ghi điểm vào ô "Điểm thi".

Section 15: For candidates who wish to be exempted from the Foreign Language exam, they need to clearly state the type of certificate that qualifies for exemption or specify that they are members of the national team participating in the International Olympic Foreign Language exam as regulated by the Ministry of Education and Training. For certificates with test scores (overall test scores), candidates must enter the score in the "Test Score" box.

- Section 16: Candidates who took the high school graduation exam in the previous year, if they have exams/subjects that qualify for retention as regulated, and they want to retain the scores of which exam/subject, they need to enter the score of that exam/subject in the corresponding box (Note: For combination exams, candidates are allowed to retain the results of each component subject if qualified, to retain the overall score of which combination exam, candidates must enter the scores of all component subjects of that combination exam).

For exams/subjects that are retained, candidates can still register to take the exam (the overall exam or the component subject) in Section 14 only if they have the desire to use the exam results for university admission.

- Sections 24, 25, 26, 27 on the software: Candidates who register to take the exam for the evaluation of admission to Early Childhood Education majors must fill in the information in these sections. For Section 24, candidates must provide complete and accurate information on the software, along with supporting documents to enjoy priority in the evaluation of admission to universities and colleges.

When is the registration period for the 2024 Vietnam High School Graduation Exam?

The Ministry of Education and Training issued Official Dispatch 1277/BGDDT-QLCL 2024 regarding the guidelines for organizing the 2024 High School Graduation Exam.

In Appendix 1 attached to Official Dispatch 1277/BGDDT-QLCL 2024, the timeline for the registration period for the 2024 Vietnam High School Graduation Exam is specified as follows:

- From April 24th to April 26th, 2024: Creating and assigning accounts for students who are currently in 12th grade during the academic year 2023-2024.

- From April 24th to April 28th, 2024: Organizing trial registration (pre-registration) for students who are currently in 12th grade online on the QLT System.

- From May 2nd to 5:00 PM on May 10th, 2024: Organizing official online registration for students who are currently in 12th grade.

Therefore, the official registration period for the 2024 High School Graduation Exam is from May 2nd to 5:00 PM on May 10th, 2024.

Additionally, students who are currently in 12th grade will be provided with registration accounts from April 24th to April 26th, 2024.
