Registration form to fulfill the obligation to participate in the People's Police according to the appendix form issued together with this Decree, with certification from the People's Committee of the commune, ward, or commune-level town (hereinafter collectively referred to as the Commune People’s Committee) where the resident resides or from the agency, organization where the individual is working or studying.

2. Military service registration certificate.
