What is the application form for re-issuance of certificates of meritorious persons and martyrs' relatives in Vietnam?
Pursuant to the provisions of Article 115 of Decree 131/2021/ND-CP stipulating dossiers and procedures for supplementing or re-issuance of certificates of meritorious people and martyrs' relatives, specifically as follows:
"Article 115. Dossiers and procedures for supplementing or re-issuance of certificates of meritorious people and martyrs' relatives
1. Individuals preparing an application using Form No. 25, Appendix I of this Decree, shall send:
a) Agencies and units that directly manage people with meritorious services serving in the army or police.
b) The commune-level People's Committee of the place of permanent residence, for people with meritorious services managed by the Labor, War Invalids and Social Affairs branch or relatives of martyrs.
2. The direct management agency or the commune-level People's Committee shall, within 5 working days, certify the application, make a list enclosed with the application and send it to the application management agency.
3. The dossier-managing agency within 12 days from the date of receipt of complete papers, shall be responsible for comparing dossiers of meritorious persons in order to supplement or re-issue certificates.
4. The Ministry of National Defense and the Ministry of Public Security shall guide the process of supplementing or re-issuance of certificates of meritorious persons for cases under management. The time for consideration and settlement shall not exceed 20 days from the date of receipt of the application.
5. Certificates of meritorious people or martyrs' relatives that were issued before July 1, 2021 are still valid. The case of re-issuance shall be made according to Form No. 102, Appendix I of this Decree."

As for the process of re-issuance the certificate of war invalids for war invalids who are in active service or working, Article 22 of Circular 55/2022/TT-BQP specifically stipulates as follows:
(1) The agency or unit directly managing the regiment level and above, within 07 days from the date of receipt of the individual's application, is responsible for examining, certifying and submitting a written request. according to decentralization (division level and equivalent within 03 days), send it to the political agency of the unit affiliated to the Ministry where the records of war invalids are managed.
(2) The political agency of the unit affiliated to the Ministry where the records of war invalids are managed, within 5 days from the date of receipt of all required documents, are responsible for comparing the dossiers and reporting to the Commander or the Political Commissar. The military zone shall re-issue the certificate of war invalids if it falls under the jurisdiction of the military zone, and transfer it to the requesting agency or unit for granting to the subject; In the remaining cases, a document enclosed with the dossier of war invalids shall be submitted to the Policy Department.
(3) The Policy Department within 05 days from the date of receipt of complete dossiers as prescribed is responsible for checking; The Director of the Policy Department re-issues the certificate of war invalids; transfer the certificate of war invalids and dossiers to political agencies of units under the Ministry for management and implementation.
