What are the instructions for filling out the registration form for participating in Police duty in Vietnam?

Based on the Appendix issued together with Decree 70/2019/ND-CP providing instructions for filling out the registration form for participating in Police duty in Vietnam as follows:

- (1) Color photographs (4x6 cm) taken within 06 months from the date of declaration, affixed and stamped.

- (2) Full name and birth name: Write capital letters in accordance with the first and last name on the Birth Certificate.

- (3) Date of birth: Write down the full date, month and year of birth exactly as shown on the Birth Certificate.

- (4) Place of birth: Write down the name of the commune (or ward, town), district (or district, town, provincial city), province (or centrally-run city) where the declarant was born (as information on the Birth Certificate). If there is a change in the name of the administrative unit, write down both the old name and new name.

- (5) Hometown: Record according to the birth certificate or ID card or citizen ID card (the determination of the hometown shall comply with the law and the guidance of the Ministry of Justice); specify the name of the commune (or ward, town), district (or district, provincial city), province (or centrally-run city).

Application
Citizens participating in the selection for the Police duty shall submit the following documents to the Police authorities of communes, wards and towns (hereinafter collectively referred to as commune-level police authority) where citizens register permanent residence:
1. The registration form for participating in Police duty according to the form in the Appendix issued together with this Decree certified by the People's Committee of the commune, ward or town (hereinafter collectively referred to as the commune-level People's Committee) of the place of residence or agency, organization where the applicant is working and studying.
2. Certificate of registration of military service.

Thus, according to the above provisions, the application for participating in Police duty in Vietnam includes:
