Pursuant to Section 2 of the List issued together with Decision 3191/QD-BCA in 2022, the list of administrative procedures is amended and supplemented under the jurisdiction of the Ministry of Public Security, the implementation of the report of loss of universal households shall be carried out in the following 04 units:

- Immigration Department (administrative procedures carried out at the central level);

- Provincial Immigration Authority (administrative procedures carried out at the provincial level);

- District-level Immigration Authority (administrative procedures carried out at the district level);

- Commune-level Immigration Authority.

* Requirements and conditions for reporting loss of ordinary passport:

Pursuant to the provisions of Subsection A Section I, Subsection A Section II, Subsection A Section III, Subsection A Section IV Procedures issued together with Decision 3191/QD-BCA in 2022, the conditions for reporting loss of ordinary passport are as follows:

- In case of reporting to the Immigration Department: Within 48 hours from the time of detection of the lost passport, the person who lost the passport directly submits or sends the passport loss report form to the Immigration Department. In case due to force majeure, the time limit for submitting or submitting a loss notice may be longer, but in the application, a specific explanation of the reason for force majeure must be explained.

- In case of reporting to the Provincial Immigration Agency: Within 48 hours from the time of detection of the lost passport, the person who lost the passport directly submits or sends the passport loss report in the form to the Immigration Authority where it is convenient. In case due to force majeure, the time limit for submitting or submitting a loss notice may be longer, but in the application, a specific explanation of the reason for force majeure must be explained.

- For cases reported to the district-level Immigration Authority: Within 48 hours after discovering the lost passport, the person who lost the passport shall directly submit a report of loss of passport in the form to the district police. In case due to force majeure, the time limit for submitting or submitting a loss notice may be longer, but in the application, a specific explanation of the reason for force majeure must be explained.

- For cases reported to the commune-level Immigration Authority: Within 48 hours from the time of detection of the lost passport, the person who has lost his/her passport shall directly submit an application for loss of passport in the form to the commune-level police. In case due to force majeure, the time limit for submitting or submitting a loss notice may be longer, but in the application, a specific explanation of the reason for force majeure must be explained.

(i) 01 application for loss of ordinary passport (form TK05) issued together with Circular 73/2021/TT-BCA; For children under 14 years old, people who have lost civil act capacity, people with cognitive difficulties, mastering the act of losing their passport, the report of loss of passport is issued by the father, the mother or legal representative declares and signs on her behalf. In case of sending a loss report via the postal service, the passport loss report must be confirmed by the Chief of Police of the ward, commune or town where the citizen is residing or the place where the passport is reported to be lost.

(ii) Documents proving that they are the parent or legal representative in case of reporting the loss of passports to children under 14 years of age, persons who have lost civil act capacity, people with difficulties in perception and behavior control.

+ Name of application form, declaration form: Report of loss of ordinary passport (form TK05) issued together with Circular 73/2021/TT-BCA.
