- A request form for converting a driver's license (according to Form No. 05);

- A copy of the labor contract (for contract laborers); a copy of the identity card for officers, professional soldiers, workers, public employees in national defense, military ID card or a copy of one of the following documents: Military enlistment decision, recruitment decision, promotion decision, latest salary increase decision that is still valid.

- A decision assigning the task of driving, signed by the head of the unit from regiment level and equivalent or higher, not more than 06 months from the date of signing (not applicable to officers changing licenses to perform missions);

- A certificate of completion of the training program for converting to a military driver's license issued by a military driving training institution;

Step 2: The Vehicle-Machine departments of the units, according to the assigned levels, will check the dossier, consolidate and report the list (with the dossier) to the focal Vehicle-Machine department under the Ministry of National Defense for consolidation and reporting to the Vehicle-Machine Department.

Step 3: From the date of receiving a complete dossier as per regulations, within 15 working days, the Vehicle-Machine Department is responsible for reviewing, appraising, and converting the driver's license as per regulations. In case the conditions for converting the driver's license are not met, the Vehicle-Machine Department will notify in writing, clearly stating the reasons.
