According to Article 11 of Joint Circular 43/2011/TTLT-BLDTBXH-BYT, which stipulates the procedures for transferring individuals who have completed drug rehabilitation back to their families and communities:

When the rehabilitation period ends, the rehabilitation facility must:
1. Issue a certificate of completion of drug rehabilitation, according to the form specified in Appendix 8 attached to this Circular.
2. Transfer the individual who has completed rehabilitation to their family or guardian.
3. Send a copy of the certificate of completion of drug rehabilitation; assessment results of rehabilitation, recovery; and any documents of rule violations during the rehabilitation period (if any) to the commune-level People's Committee of the place where the individual resides.
Upon the completion of the rehabilitation period, the rehabilitation facility has the following responsibilities:
- Issue a certificate of completion of drug rehabilitation.- Transfer the individual who has completed rehabilitation to their family or guardian.- Send a copy of the certificate of completion of drug rehabilitation; assessment results of rehabilitation, recovery; and any documents of rule violations during the rehabilitation period (if any) to the commune-level People's Committee of the place where the individual resides.

Where can drug addicts register for voluntary drug rehabilitation?
According to Clause 1, Article 28 of Decree 116/2021/ND-CP:
Registering for voluntary drug rehabilitation and opioid substitution treatment
1. Within 05 working days from the date the competent authority establishes the drug addiction status, drug addicts aged 12 years and older (hereinafter referred to as drug addicts) must register for voluntary drug rehabilitation or opioid substitution treatment at the commune-level People's Committee where they reside. In cases of individuals who do not have a stable place of residence, they should register at the commune-level People's Committee of the area where they committed the legal violations and undergo rehabilitation at a public drug rehabilitation facility following a decision by the competent authority.
2. The procedures and documentation for registration for opioid substitution treatment shall be implemented in accordance with the Government of Vietnam. Within 05 working days of the drug addict's participation or termination of treatment, the opioid substitution treatment facility must notify the Chairman of the commune-level People's Committee where the addict registered for supervision or initiate the process of mandatory rehabilitation application.
3. Locations for voluntary rehabilitation:
a) At the family, community;b) At public drug rehabilitation facilities or voluntary drug rehabilitation facilities.
4. The registration dossier for voluntary rehabilitation includes:
a) 01 voluntary rehabilitation registration form according to Form 22 of Appendix II of this Decree;b) 01 copy of the result certificate confirming the drug addiction status issued by the competent authority;c) 01 photocopy of one of the following identification documents: citizen identification card or identity card, passport, birth certificate (for drug addicts aged 12 to under 18 years) of the drug addict.
5. The procedure for voluntary rehabilitation registration:
a) The drug addict or the legal representative of a drug addict aged 12 to under 18 years submits 01 registration dossier as stipulated in Clause 4 of this Article at the voluntary rehabilitation registration reception point of the commune-level People's Committee; and presents the original identification documents for verification.
b) The receiver shall verify the dossier, compare identification documents and enter the registration details into the voluntary drug rehabilitation register according to Form 23 of Appendix II of this Decree. In case the dossier is incomplete, the receiver shall guide the registrant to complete and supplement the voluntary rehabilitation registration dossier.
According to the above regulations, drug addicts aged 12 years and older must register for voluntary drug rehabilitation at the commune-level People's Committee where they reside.
In cases of individuals who do not have a stable place of residence, they should register at the commune-level People's Committee of the area where they committed the legal violations and undergo rehabilitation at a public drug rehabilitation facility following a decision by the competent authority.
How is the acceptance of voluntary rehabilitation registration conducted?
According to Article 29 of Decree 116/2021/ND-CP:
Acceptance of voluntary rehabilitation registration
1. The Chairman of the commune-level People's Committee is responsible for arranging the location and personnel to receive documents and guide the voluntary rehabilitation registration.
2. The voluntary rehabilitation registration reception point must be equipped with the necessary facilities for welcoming and storing registration documents.
Thus, the acceptance of voluntary rehabilitation registration is conducted as follows:
- The Chairman of the commune-level People's Committee arranges the location and personnel to receive documents and guide the voluntary rehabilitation registration.- The voluntary rehabilitation registration reception point must be equipped with the necessary facilities for welcoming and storing registration documents.
