c) Procedure for reconsidering and reissuing a lost membership card, or replacing a damaged membership card
- The Party cell, based on the reason for the loss or damage of the card stated in the member’s self-review report, examines and collects 2 portrait photos (size 2 x 3 cm) along with the list requesting the grassroots Party Committee. The grassroots Party Committee reviews and submits the list (using forms 2-TDV and 3-TDV) requesting the authorized Party Committee. The organizational department of the authorized Party Committee compiles the list (using forms 2A-TDV and 3A-TDV), reports to the Standing Committee of the Party Committee for consideration, and issues a decision to reissue the lost membership card or replace the damaged membership card (using form 4-TDV).
- After the decision of the Party Committee, the organizational department of the Party Committee completes the list of reissued membership cards into the Party member database management software. If there is no network connection to the provincial server, data is extracted to a file, submitted to the organizational department of the Provincial Party Committee and equivalents; photos of Party members are sent to the organizational department of the Provincial Party Committee and equivalents; the organizational department of the Provincial Party Committee and equivalents uses the Party member database management software to approve and print the membership cards, affix photos, emboss seals, and deliver the cards to the District Party Committee and equivalents for completion and distribution to the Party cell for issuance to members.

Thus, the procedure for reissuing a lost Communist Party membership card is as follows:
Step 1: The Party cell reviews the reason for losing the membership card and submits a dossier including 2 portrait photos (size 2 x 3 cm) and the list requesting the grassroots Party Committee.
Step 2: The organizational department of the authorized Party Committee compiles the list, reports to the Standing Committee of the Party Committee for consideration and decision to reissue the lost membership card.
Step 3: The organizational department of the Party Committee completes the list of reissued membership cards into the Party member database management software.

Article 29 of Regulation 69-QD/TW in 2022 stipulates the violations in organizational and personnel work as follows:
Violations in Organizational and Personnel Work
1. A Communist Party member violating one of the following cases causing minor consequences shall be disciplined by reprimand:
a) Interfering, influencing the competent organizations or individuals to elect, appoint, plan, appoint, re-appoint, transfer, dismiss, resign, nominate, apply, study, obtain titles, ranks, recognition of standard titles, promotion exams, salary increases, going abroad, implementing policies contrary to regulations.
b) Directing or requesting the appointment of individuals who do not meet the conditions and standards according to regulations.
c) Not complying with principles, procedures, and processes in personnel work; not adhering to regulations on personnel work; falsifying or arbitrarily modifying Party members' documents, officials and public employees' documents; assessing and evaluating officials without basis, dishonestly, and non-objectively.
d) Not complying with appointment, transfer, rotation, work assignment, retirement policies, sending for study, transferring Party activity according to regulations.
e) Implementing appraisal, advising, proposing, deciding on reception, recruitment, examination, staff allocation, rotation, reward, promotion titles, ranks without meeting the necessary standards and conditions.
f) Directing or appraising, advising, proposing, implementing recruitment, reception, work arrangement, appointment, promotion, recognition of titles, rotation, transfer, raising grades, rewarding, disciplining improperly against principles, procedures, regulations; planning, training, sending people to work abroad without meeting the necessary standards.
g) Losing a membership card without a legitimate reason; evaluating, recommending acceptance and recognizing an official Party member who does not meet the standards, conditions, processes, and procedures; improperly implementing Party activity transfer for Party members or submitting non-compliant Party activity transfer documents; not completing the Party activity transfer report for Party members; incorrectly confirming Party membership age.
h) Reporting, creating dossiers, declaring personal and family background not fully, dishonestly. Declaring Party membership record incorrectly regarding personal political characteristics and family relations.

3. In cases of violating regulations in Clauses 1 and 2 causing severe consequences or violating one of the following cases, the discipline shall be expulsion:
a) Brokering or receiving bribes in reception, recruitment, appointment, re-appointment, job allocation, promoting salaries, ranks, rewarding, disciplining, recognizing standards, or awarding titles to officials, Party members, officials and public employees, workers.
b) Engaging in activities to extend service age, work experience, titles, discipline, degrees, job positions, rotation for personal or others' benefit.
c) Abusing power, positions, or office to create conditions, influence, and interfere with reception, recruitment, planning, rotation, appointment, job placement improperly.
d) Falsifying or altering documents to study, to be recruited into organizations.
e) Using membership cards, professional cards, official cards to mortgage, pledge for loans, or assets.
Thus, if a Communist Party member loses their membership card without a legitimate reason, they will be reprimanded. If the loss of the membership card without a legitimate reason causes serious consequences, they will be expelled from the Communist Party.
What Does a Party Membership Dossier Include When Being Considered for Admission into the Communist Party?
Section 1.1, Subsection 1, Section II of Guidance 12-HD/BTCTW in 2022 outlines the list of documents required in a Party membership dossier when being considered for admission into the Communist Party:
(1) Certificate of completion of the class on Communist Party awareness;
(2) Application for Communist Party membership;
(3) Biography of the applicant and accompanying verification documents;
(4) Introduction letter from the appointed official Party member offering assistance;
(5) Resolution introducing the outstanding member to the Communist Party from the Ho Chi Minh Communist Youth Union or the grassroots trade union (if any);
(6) Summary of comments from political-social organizations where the applicant resides and the cell (or Party cell) where they live for the applicant;
