For the maternity policies of those currently paying social insurance: The dossier is according to the provisions of Article 101 of the Social Insurance Law; Clauses 1, 2, 3, 4, 5 of Article 5 of Decree No. 115/2015/ND-CP; Articles 15, 18, 21 of Circular No. 56/2017/TT-BYT; Article 7 of Decree No. 33/2016/ND-CP and Clause 2 Article 15 of Decree No. 143/2018/ND-CP, including Form 01B-HSB prepared by the employer and the following documents:

2.2.1. Female employees taking prenatal check-ups, experiencing miscarriages, abortions, suction abortions, stillbirths, or pathological abortions; employees undergoing contraceptive measures:

a) For inpatient treatment: A copy of the hospital discharge certificate of the employee; in cases of referral during inpatient treatment, an additional copy of the referral document or hospital transfer document is required.

b) For outpatient treatment: A certificate of leave for social insurance; or a copy of the hospital discharge certificate with an indication from the treating doctor for additional leave post-inpatient treatment.

Thus, employees, including women wanting to take leave for prenatal check-ups covered by social insurance, need to obtain a certificate of leave for social insurance after each check-up. This document is also known as the prenatal check-up certificate for social insurance.
