The Latest Application Form for Granting Citizen Identification Card?
According to the provisions of Clause 1, Article 7 Circular 66/2015/TT-BCA (amended by Clause 4, Article 1 Circular 41/2019/TT-BCA), the Application Form for Granting Citizen Identification Card is Form CC01, as follows:
Form CC01 is used for citizens to declare information about their identity when requesting the issuance, renewal, or reissuance of the Citizen Identification Card (including cases where citizens fill out information on the online public service portal).

Instructions on How to Fill Out the Latest Application Form for Granting Citizen Identification Card?
According to the provisions of Clause 2, Article 7 Circular 66/2015/TT-BCA (amended and supplemented by Clauses 5, 6, 7, and 8, Article 1 Circular 41/2019/TT-BCA), the way to fill out the Citizen Identification Card application form is as follows:
- Section “Surname, Middle Name, and Given Name”, “Other Names”: fill in the full surname, middle name, and given name as per the birth certificate; write in uppercase letters with full diacritics. Only fill in the surname, other names if the birth certificate includes the surname and other names;
- Section “Date of Birth”: fill in the date of birth of the citizen to be issued, renewed, or reissued the Citizen Identification Card. The day should be written in 02 digits; the year should be written in full four digits. For months from March to September, write in 01 digit, other months write in 02 digits;
- Section “Gender”: if male, write “Male”, if female, write “Female”;
- Section “Ethnicity”, “Religion”: fill in the ethnicity and religion of the citizen to be issued, renewed, or reissued the Citizen Identification Card as per the birth certificate or certificate of ethnicity or religion issued by competent authority;
- Section “Nationality”: fill in the nationality of the citizen to be issued, renewed, or reissued the Citizen Identification Card as per the birth certificate or document proving Vietnamese nationality issued by a competent authority;
- Section “Marital Status”: fill in the current marital status of the citizen to be issued, renewed, or reissued the Citizen Identification Card, including: single, married, or divorced;
- Section “Blood Type” (if any): fill in according to the blood type test result of the citizen requesting the issuance, renewal, or reissuance of the Citizen Identification Card;
- Section “Place of Birth Registration”: Fill in the administrative location at the commune, district, provincial level as per the birth certificate of the citizen. If the birth certificate does not fully list the administrative location at the commune, district, provincial level, then fill in according to the birth certificate. If there is a change in the administrative location, fill in according to the new modified administrative location;
- Section “Hometown”: Fill in the administrative location at the commune, district, provincial level as per the birth certificate, household registration book. If these documents do not fully list the administrative location at the commune, district, provincial level, then fill in according to these documents. If there is a change in the administrative location, fill in according to the new modified administrative location;
- Section “Permanent Residence”: fill in fully and accurately according to the household registration book. In case the citizen requesting the issuance, renewal, or reissuance of the Citizen Identification Card is in official service in the People's Army, People's Police and is residing collectively in barracks, collective housing, fill in according to the introduction letter of the agency/unit issued to the citizen;
- Section “Current Residence”: fill in fully, clearly, accurately the current residence of the citizen in the order of house number, street; village, hamlet, alley, block, farm; commune/ward/commune-level town; district/district-level town/provincial city; central city/province;
- Section “Occupation”: clearly state the current occupation; if the citizen is a serving soldier, leave blank. Section “Educational Level”: clearly state the highest educational level (PhD, master's, university, college, intermediate, high school graduate, secondary school graduate...);
- Sections 17, 18, 19, 20, 21: fill in fully the surname, middle name, given name; nationality; Citizen Identification Number or Identity Card Number in the respective sections in the form (if any);
- Section “Citizen’s Request”:
+ “Issuance, Renewal, Reissuance of Citizen Identification Card”: For the first-time issuance, state new issuance; for cases of damage, expiration, or change, error in information on the Citizen Identification Card or citizens requesting a new card, state renewal; for cases of loss of card or returning to Vietnamese nationality, state reissuance;+ “Delivery of Citizen Identification Card to citizen’s address”: If the citizen requests the delivery of the Citizen Identification Card, state “yes” and fill in the full address, phone number for contact; if not requested, state “no”;- Section “Date....Month……..Year……”: clearly state the date, month, year the citizen fills out the application form for issuance, renewal, reissuance of the Citizen Identification Card.
- The two-dimensional barcode part encrypts all personal information of the citizen and the management information on the Online Citizen Identification Card Application Form.
Section “Appointment Time”: Register the specific date the citizen comes to the Citizen Identification Card management agency to complete the procedures for issuing, renewing, reissuing the Citizen Identification Card; section “At”: Fill in the name of the Citizen Identification Card management agency the citizen registers to come to complete the procedures for issuing, renewing, reissuing the Citizen Identification Card.
Note: Section "Verification Result”: if the dossier requesting the issuance, renewal, or reissuance of the Citizen Identification Card requires verification through the citizen identification archive, the receiving unit requests the Citizen Identification Archive Team under the Administrative Management of Social Order Police Division to compare, verify with the original dossier and respond with the result to the requesting unit.
Who is Eligible to be Issued a Citizen Identification Card?
Article 19 of the Law on Citizen Identification 2014 stipulates the subjects eligible for a Citizen Identification Card as follows:
- Vietnamese citizens aged 14 and older shall be issued with a Citizen Identification Card.
- The Citizen Identification Number is the personal identification number.
