Latest Periodic Health Examination Record Form?
On May 5, 2023, the Minister of Health issued Circular 09/2023/TT-BYT amending Circular 14/2013/TT-BYT guiding health examinations issued by the Minister of Health.

What does the latest periodic health examination record include?
Pursuant to Article 4 Circular 14/2013/TT-BYT (amended by Clause 1 Article 1 Circular 09/2023/TT-BYT) on periodic health examination records as follows:
Health Examination Record
1. Health examination record for individuals aged 18 (eighteen) years or older is the Health Examination Certificate in the form specified in Appendix 1 issued together with this Circular, with a 04cm x 06cm portrait photo taken on a white background within 06 (six) months up to the date of submitting the health examination record.
2. Health examination record for individuals under 18 (eighteen) years of age is the Health Examination Certificate in the form specified in Appendix 2 issued together with this Circular, with a 04cm x 06cm portrait photo taken on a white background within 06 (six) months up to the date of submitting the health examination record.
3. For individuals who are incapacitated for civil acts or incapable of civil acts or restricted in civil act capacity requesting health examination but not in the case of periodic health examination, the health examination record includes: Health Examination Certificate as specified in Clause 1 or Clause 2 of this Article and written consent from the parent or legal guardian of that individual.
4. For individuals undergoing periodic health examinations, the health examination record includes:
a) Periodic health examination record form as specified in Appendix 3 issued together with this Circular;
b) Introduction letter from the agency or organization where the individual is working if the periodic health examination is conducted individually or the individual's name is included in the list of periodic health examinations confirmed by the agency or organization where they are working to carry out the periodic health examination under contract.
The periodic health examination record for employees includes:
- Periodic health examination record form as specified in Appendix 3a issued together with Circular 09/2023/TT-BYT- Introduction letter from the agency or organization where the individual is working if the periodic health examination is conducted individually or the individual's name is included in the list of periodic health examinations confirmed by the agency or organization where they are working to carry out the periodic health examination under contract.
How is health classification done during health examinations?
Based on Article 7 Circular 14/2013/TT-BYT, the regulations are as follows:
- The individual conducting the clinical and subclinical examination for the health examination subject must clearly record the examination results, health classification of specialized fields, sign, and take responsibility before the law for their examination and examination results.
- Based on the examination results of each specialized field, the individual appointed by the health examination unit to conclude the health classification and sign the Health Certificate, Periodic Health Examination Record (hereinafter abbreviated as the conclusion person) performs the health classification conclusion as follows:
+ Classify the health of the individual undergoing health examination as per Decision 1613/BYT-QD or according to the professional health standards set for specialized health examinations;
+ Clearly state the diseases and disabilities of the individual undergoing the health examination (if any). If the individual undergoing the health examination has diseases or disabilities, the conclusion person must advise on treatment plans, functional rehabilitation, or refer to a specialist for diagnosis and treatment.
- After classifying the health, the conclusion person must sign, clearly state their full name and affix the seal of the health examination unit on the Health Certificate or Periodic Health Examination Record (the seal used in official transactions of the health examination unit as per the legal regulations on the management and use of seals).
If the individual undergoing the health examination requests multiple Health Certificates, the sealing is done after replicating the Health Certificates as per Article 8 Circular 14/2013/TT-BYT.
Circular 09/2023/TT-BYT will take effect from June 20, 2023.
