If I lose my health insurance card, what documents should I submit to get a new card?
Pursuant to the provisions of Clause 1, Article 18 of the 2008 Law on Health Insurance of Vietnam and subsection 2, Section A, Part II of the Amendment and Supplementation Procedures promulgated together with Decision 62/QD-BYT in 2023, the health insurance card is invalid. If the insured person dies, the insured person will prepare the necessary papers and documents to submit to the social insurance agency in order to be re-issued the health insurance card.
According to that, the application for re-issuance of the health insurance card when the health insurance card is lost includes:
(1) Declaration of participation and adjustment of health insurance information issued by Vietnam Social Insurance;
(2) The receipt of the application and the appointment to return the results of the re-issuance of the health insurance card according to Form No. 4 of the Appendix issued together with Decree 146/2018/ND-CP as amended and supplemented by Article 2 of Decree 104/2022/ND-CP.
Thus, as soon as it is discovered that the health insurance card is lost, the insured person shall make the above dossier.

Pursuant to the provisions on the re-issuance of the health insurance card in Article 18 of the 2008 Law on Health Insurance of Vietnam (amended by Clause 12, Article 1 of the 2014 Law on Amendment to the Law on Health Insurance of Vietnam and point d, Clause 2, Article 23 of the 2015 Law on Fees and Charges of Vietnam) as follows:
Re-issuance of health insurance cards in Vietnam
1. Health insurance cards may be re-granted to replace the lost ones.
2. A person who loses his/her health insurance card shall file a written request for the re-grant of the card.
3. The insured shall be re-issued with the health insurance cards by the health insurance organizations within 7 working days from the receipt of the written requests for the re-issuance of such cards. During the processing time for the issuance of the health insurance cards, the insured are eligible to receive the health insurance benefits.
Thus, according to the above regulations, the insured shall be re-issued with the health insurance cards by the health insurance organizations within 7 working days from the receipt of the written requests for the re-issuance of such cards.
What is the procedure for renewal of a health insurance card on the National Public Service Portal?
In addition to carrying out the procedures for re-issuance of health insurance directly at the one-stop section of the social insurance agency of the province or district according to the provisions of subsection 2, Section A, Part II, the procedures for amending and supplementing the Decision issued together with Decision 62/QD-BYT in 2023, health insurance participants can also carry out procedures for re-issuance of health insurance cards online at home through the National Public Service Portal.
Specifically, the steps to apply for a new health insurance card online on the National Public Service when losing your health insurance card are as follows:
Step 1: Persons who have lost their health insurance cards access the homepage of the National Public Service Portal through the address <https://dichvucong.gov.vn/p/home/dvc-trang-chu.html>;
Step 2:
- If you already have an account, click the "Login" button on the right corner of the homepage of the National Public Service Portal;
- In case you do not have an account, click the "Register" button to register an account and log in to the National Public Service Portal;
+ When registering, the participants of health insurance need to choose the registration method as "Citizen".
+ Select the logo "Social insurance";
+ Enter all necessary personal information including: Username (ID card/ citizen ID card number); Social insurance code; Full name; Date of birth; Phone number; Email (if any);
+ Then enter the verification code and press the "Register" button;
+ Create a password for the account and complete the account registration process on the National Public Service Portal.
Step 3:
Click on the account name on the National Public Service Portal in the right corner of the screen, select Personal information.
The system will automatically redirect to the account's information page;
Step 4:
At the Personal Information page, click on the "Utilities" -> "Social Insurance" bar;
At that time, the social insurance information will appear -> Enter the social insurance code -> Click on the "Update" box;
Step 5:
Return to the homepage of the National Public Service Portal -> Click "Online Public Service" in the lower left corner of the search bar;
Step 6:
The online public service implementation page will appear. At this time, click to select the agency that carries out the procedure as "Vietnam Social Insurance" -> Click "Search".
The public service system of Vietnam Social Insurance will appear -> Click "Re-issue, change, adjust information on social insurance book, health insurance card" -> Select re-issue content health insurance card due to damage or loss -> Click "Submit online";
Step 7:
The system will automatically navigate to the online Public Service Portal interface of Vietnam Social Security. The information requested for re-issuance of the health insurance card will be automatically displayed.
At the Address to receive results, there are 02 forms as follows:
- The department receives dossiers and returns results;
- Via postal service.
Select the appropriate form and complete the procedure for re-issuance of the health insurance card due to the loss of the card.
