GUIDANCE ON FILLING OUT SOME INFORMATION IN THE INFORMATION SHEET FORM (1) Enter the name of the Department of Natural Resources and Environment, the Land Registration Office, or the branch of the Land Registration Office. (2) Enter the File Code as prescribed in Decree 61/2018/ND-CP dated April 23, 2018, on implementing administrative procedures under the single-window system on the Receipt and Appointment Note (using Form 01 issued together with Circular 01/2018/TT-VPCP dated November 23, 2018, from the Office of the Government of Vietnam guiding the implementation of some provisions of Decree 61/2018/ND-CP dated April 23, 2018, of the Government of Vietnam on implementing the single-window system in solving administrative procedures). (3) Enter according to the time of receiving the dossier on the Receipt and Appointment Note. Section II. Enter information about the land user, the owner of the property attached to the land. If there are multiple co-owners of land use rights or the right to own property attached to the land, enter the information of one representative. (4) For individuals, enter the full name and year of birth; for households, enter the names and years of birth of household members who share land use rights; for married couples, enter the full names and years of birth of both the husband and wife; for organizations, enter the name according to the establishment decision or business registration certificate, investment license; for foreign individuals and overseas Vietnamese, enter the full name, year of birth, and nationality. (5) Address for sending notifications of financial obligations and contact in case it is necessary to request additional documents as required. (6) Enter the type of personal/legal document such as Citizen ID, Identification Card, Passport, Establishment Decision, etc. (7) Enter the type of procedure such as: Issuance of a first-time Certificate, transfer, gifting, etc. Section III. Item 3.1. Enter information for each parcel. If there are multiple parcels, positions, legal bases, or purposes of use, enter the general information and total area; also, create a list according to Form 01a/LCHS. (8) State the street/road name, ward/commune-level town, district/district-level town/city; (9) Parcel position in the land price list (position 1, 2, 3, ...); (10) Purpose of land use according to land classification and the purpose for which the land levy, land rent, or purpose after changing land use purpose is calculated. (11) Land price according to the price list or specific land price; (12) Enter the type of document, number, date, month, year, and summary of the document. For example: Land allocation decision No. 15/QD-UBND dated June 28, 2016, regarding the allocation of resettlement land, etc. Item 3.2. Enter information about property attached to the land. Section V. (13) Enter documents submitted by the land user, property owner. |

Based on Article 31 of the Land Law 2024, the current obligations of all land users are as follows:

1. Use land for the correct purpose, within the boundaries of the parcel, conform to the depth of use underground and the height in the air, protect public works underground, and comply with other relevant legal provisions.
2. Declare land registration; complete all procedures in case of conversion, transfer, lease, sublease, inheritance, gifting of land use rights, mortgage, capital contribution by land use rights as prescribed by law.
3. Fulfill financial obligations regarding land as prescribed by law.
4. Implement measures to protect land; treat, renovate, and restore land in areas affected by pollution or degradation caused by themselves.
5. Comply with environmental protection regulations, do not harm the assets and lawful interests of related land users.
6. Comply with legal regulations regarding the discovery of objects underground.
7. Handover land when the State makes a land recovery decision as prescribed by law.

What are the forms of exploitation and use of land-related information and data in Vietnam?
Based on Article 59 of the Decree 101/2024/ND-CP, the forms of exploitation and use of land-related information and data in Vietnam are carried out as follows:
- Online exploitation on the national land information portal, the national public service portal, the public service portal of the Ministry of Natural Resources and Environment, provincial public service portals, via SMS services, web service and API services; exploitation through utilities, applications that create products, value-added from the national land database as prescribed.
- Direct exploitation at the headquarters of the land data supply agency or the Department of Receiving and Returning Administrative Procedure Results.
- Exploitation through other forms as prescribed by law.
