- Proposal for appointment signed and sealed by the collective leadership of the unit.- Minutes of Meetings attached with vote count minutes at each step of the appointment process.- Curriculum vitae of the public employee self-declared according to the prescribed form, confirmed by the directly managing public service provider, with a color photo size 4 cm x 6 cm, taken within the last 06 months; Brief profile of the staff (according to the attached form).- Self-evaluation report for the last 3 years of work.- Comments and assessments by the collective leadership of the unit on moral qualities, lifestyle, sense of organizational discipline, internal solidarity, work capacity, and performance results in the past 3 years.- Comments from the local party cell where the individual and their family reside according to regulations.- Conclusion of the competent party committee on the political standards of the personnel.- Written comments and evaluations by the Unit Party Committee on the personnel proposed for appointment.- Asset and income declaration according to the prescribed form.- Copies of diplomas and certificates as required by the appointment standards; In the case of diplomas issued by foreign institutions or domestic institutions linked with foreign institutions, they must be certified by the Ministry of Education and Training.- Written consent from the competent authority managing the public employee (for personnel sources from other locations).- 5-year term action plan.- Health certificate issued by a competent health facility within 06 months.

Pursuant to the regulations at Subsection 11 Section VIII Part B administrative procedures issued together with Decision 3096/QD-BYT in 2023, the procedures for transferring leadership and management positions of subordinate units under the Ministry of Health managed by the Party Personnel Committee are as follows:
Step 1. Based on specific task requirements, planning, and usage plan of officials and public employees; the Party Personnel Committee considers and decides to transfer and receive public employees from units outside the Ministry of Health, public employees from one public service provider to another within the Ministry to appoint to management positions:
In case the proposed personnel by the Party Personnel Committee comes from outside the affiliated public service provider: After obtaining a resolution by the Party Personnel Committee approving the policy, the Organization and Personnel Department of the Ministry of Health proceeds as follows:
- Consult the collective leadership and party committee of the public service provider receiving the personnel on the Party Personnel Committee's proposed appointment.- Consult the collective leadership and party committee where the personnel is currently working about the appointment policy and take comments and evaluations from the current leadership and party committee; study the dossier, and verify the personnel's background.- Meet with the proposed personnel to discuss work task requirements.- Host and cooperate with related agencies and units to verify personnel.
Step 2. The party committee and leadership of the public service provider request the conclusion of the competent party committee on the political standards of the proposed personnel, complete the appointment dossier, and send it to the Party Personnel Committee.
Step 3. The Organization and Personnel Department reviews the appointment dossier and reports to the Party Personnel Committee.
Step 4. The Party Personnel Committee considers and decides.
Step 5. The Organization and Personnel Department drafts the appointment decision for the Minister of Health to sign and issue.
Step 6. Discussions at meetings are recorded in minutes. Conduct vote counting and prepare vote count minutes at the unit.
Participants in the vote counting include representatives of the unit's collective leadership, representatives of the unit's Organization and Personnel Department, and representatives of the leadership or staff of the Organization and Personnel Department of the Ministry of Health (if any).
Step 7. Counted ballots are sealed with signatures from the unit's collective leadership, representatives of the Organization and Personnel Department, witnesses (if any), and stored at the unit.
Conditions for Transferring Leadership and Management Positions of Subordinate Units under the Ministry of Health Managed by the Party Personnel Committee
Pursuant to the regulations at Subsection 11 Section VIII Part B administrative procedures issued together with Decision 3096/QD-BYT in 2023 the conditions for transferring leadership and management positions of subordinate units under the Ministry of Health managed by the Party Personnel Committee are as follows:
- Completed the tasks during the term of holding a leadership and management position.- Meeting the standards and conditions of the leadership and management position according to the competent authority's regulations at the time of extension as prescribed in Articles 6, 7, 8 of Decision 4468/QD-BYT.- The agency or organization has a demand for the leadership and management position.- Being in good health to fulfill the assigned duties and responsibilities.- Not being in cases prohibited from holding positions according to the law.
