(3) Copy, electronic copy, or original of the list of drivers transporting hazardous goods (including: full name; identification card number or citizen identification number or passport number; license number, license class (for drivers);certificate of special competency, class (for crew members) and the list of escorts if mandatory (including: full name, identification card number or citizen identification number or passport number) accompanied by a copy of the safety training completion certificate as required;
(4) Copy, electronic copy, or original of the certificate indicating completion of training for road motor vehicle operators transporting hazardous goods by road;copy, electronic copy, or original of the special professional certificate for crew members transporting hazardous goods on inland waterways;
(5) Copy, electronic copy, or original of the transportation plan for hazardous goods of the transporting unit according to the form specified in Appendix V of Decree 34/2024/ND-CP (including at least the following contents: vehicle registration number; full name of the driver; type of goods; mass; route, transport duration; emergency response measures in case of fire or explosion);copy, electronic copy, or original of the oil spill response plan (only applicable for the transport of petroleum on inland waterways).

- Step 1: Submit the application
+ The hazardous goods transporter submits 01 set of application documents for the issuance of a licence to transport dangerous goods of Class 1, Class 2, Class 3, Class 4, and Class 9 as per regulations in Clause 2, Article 18 of Decree 34/2024/ND-CP to the permit issuance authority.
+ For direct submission: The administrative procedure processing authority checks the components of the documents and gives an immediate response when the organization or individual submits the application; if the components of the documents are incomplete or incorrect, they guide the completion as per regulations.
+ For submissions via postal service or online public service system: Within no more than 01 working day from the date of receipt of the application, the administrative procedure processing authority reviews the completeness, if incomplete as per regulations, the authority shall notify in writing or via the online public service system for the organization or individual to supplement and guide the completion as per regulations.
- Step 2: Process the application
Within 05 working days from the date of receipt of a complete application as per regulations, the permit issuance authority reviews and processes the application for the licence to transport dangerous goods.
If the permit is not granted, the permit issuance authority must respond in writing or notify via the online public service system, stating the reasons.
