According to Article 44 of Circular 32/2023/TT-BYT, the responsibilities of duty positions in hospitals are as follows:
(1) Duty Leadership:
- Subjects for Duty Leadership: The Director, Deputy Directors, and Heads of Departments/Rooms are appointed by the Director to participate in duty leadership and must be authorized in writing.
- Responsibilities of Duty Leadership:+ Inspect and urge on-duty shifts in the hospital.+ Direct the resolution of complex issues beyond the capability of lower-level duty staff.+ Direct the handling of unusual security and order incidents in the hospital.+ Report to the direct superior managing the hospital on special, unexpected occurrences that are beyond the scope of resolution.
(2) Clinical Duty:
- Organization of Clinical Duty:+ The chief of the duty shift is a healthcare professional holding the position of Head, Deputy Head of a Department, or a doctor with a postgraduate degree.+ The clinical duty doctor is a professional with sufficient expertise, capable of independently handling tasks and assuming responsibility for their solutions.+ Nurses, midwives, and medical technicians on duty are designated by the hospital director.
- Responsibilities of the Chief of the Clinical Duty Shift:+ Manage personnel during the shift.+ Examine and decide on emergency cases, new arrivals, and critically ill patients with abnormal developments that the clinical duty doctor of the department cannot resolve.+ Report and seek guidance from the duty leadership in cases beyond professional capability and special cases such as suicide, epidemics, disasters, or mass emergencies.+ Notify the security duty and report to duty leadership in cases of security breaches in the hospital.+ Inspect and urge different duty positions.
- Responsibilities of the Duty Doctor:+ Receive emergency patients.+ Monitor and handle handed-over patients.+ Guide and urge team members to execute all instructions fully.+ Assign responsibilities to each member to closely monitor and promptly manage adverse developments in critically ill patients receiving priority care.+ Examine patients and record in medical records after each visit.
- Responsibilities of Nurses, Midwives, Medical Technicians on Duty:+ The duty nurse is from each department and can organize inter-departmental nursing duties as decided by the hospital director.+ Implement doctor’s orders and provide patient care.+ Encourage patients to follow hospital regulations.+ Safeguard patient records, medicine cabinets, and departmental assets.+ Detect patients with abnormal developments or at risk of death, report to the duty doctor, and record all developments in the monitoring sheet.
(3) Paraclinical Duty:
- Organize separate duty by specialty, depending on the workload in each department.- Responsibilities of Paraclinical Duty:+ Perform techniques that support diagnosis and treatment as instructed by clinical duty.
(4) Logistics and Administration Duty:
- The pharmacy and medical equipment duty ensures adequate medicine supply and operation of medical equipment for the duty shift.- The finance-accounting duty ensures hospital fee collection for patients visiting or being admitted/discharged outside of office hours.- IT duty ensures the hospital management network runs smoothly.- Electrical and water duty ensures the functioning of generators during power outages and repairs any sudden issues.- Administrative duty ensures communication through phones and other information means (as per the medical facility's regulations).- Security duty ensures hospital security and order.- Driver duty ensures ambulances are ready for tasks when ordered.
(5) On-call Duty Outside the Hospital:
- Besides the personnel assigned to duties mentioned above, depending on each hospital’s actual conditions, the hospital leadership can arrange on-call duty outside to be ready to support in technical and administrative aspects when needed.- On-call duty personnel must maintain continuous contact during their duty time and be present at the hospital when mobilized.
What Are the Current Principles of Duty in Medical Examination and Treatment?
The principles of duty in medical examination and treatment are specified in Article 43 of Circular 32/2023/TT-BYT as follows:
- Duty outside of office hours, on holidays, and weekends must be organized by the medical facility to ensure continuous medical examination and treatment activities 24/7.- Duty shifts must be organized strictly, with full human resources, transportation, medical equipment, and medicines for patient emergencies.- Duty positions must be easily accessible and have adequate communication means.- On-duty staff must arrive before the duty start time to receive handover from the previous shift and must hand over to the next shift at the end of duty time. They must not leave their duty positions and must execute duty orders from superiors.- The duty roster is arranged monthly, published at least one week before the duty time, approved by the hospital leadership, and displayed at each duty position. Civil servants, public employees, and laborers assigned according to the duty schedule can only switch shifts at the same level with the leadership's approval.
