Ensuring the highest allowance level for grassroots healthcare personnel from July 01, 2024, upon implementing the 2024 wage reform?
Speaking at the session, regarding policies for population officers, Minister of Health Dao Hong Lan stated that the Government of Vietnam has issued Decree 05/2023/ND-CP on the implementation of preferential allowance policies for preventive healthcare workers. However, population officers are not covered by this decree.
Receiving voters' recommendations on this issue, the Ministry has directly sent delegations to the localities to understand the situation and has sent documents to the People's Committees of the provinces to review the implementation of policies for population officers, as their primary duty is to implement population policies.
According to the Minister, recently the implementation of local policies has had instances where population officers are assigned other duties but do not receive the allowances as prescribed. The Ministry has sent documents to the People's Committees of the provinces to ensure that the positions are correctly assigned according to their job titles, and in cases where they have to perform other healthcare tasks, their allowances must be ensured.
Minister Dao Hong Lan proposed that with the upcoming wage reform, agencies and sectors should pay more attention to preferential policies for grassroots healthcare personnel, including population officers, ensuring the highest allowance level according to regulations so that the healthcare workforce has adequate working conditions.

Abolishing seniority allowance for grassroots healthcare public employees from July 01, 2024?
In Resolution 27-NQ/TW of 2018, the arrangement of existing allowance policies ensures that the total allowance fund accounts for at most 30% of the total wage fund, which will be implemented as follows:
- Continue to apply concurrent position allowances; over-frame seniority allowances; regional allowances; job responsibility allowances; mobility allowances; security and defense allowances, and special allowances for armed forces (military, police, cipher).
- Combine preferential occupational allowances, job responsibility allowances, and hazardous, dangerous allowances (collectively referred to as occupational allowances) for officials and public employees of those professions and jobs with higher than normal working conditions and with appropriate state preferential policies (education and training, healthcare, courts, prosecution, civil enforcement, inspection, examination, audit, customs, forestry inspection, market management,...). Combine special allowances, attraction allowances, and long-term work support in areas with extremely difficult socio-economic conditions into allowances for working in such areas.
- Abolish seniority allowances (except for the military, police, and cipher to ensure wage correlation with officials); leadership position allowances (since leadership positions in the political system follow position-based salaries); party, union, and political-social organization work allowances; public service allowances (since they are included in the basic wage); hazardous, dangerous allowances (since the hazardous, dangerous working conditions are included in the occupational allowances).
- Set new allowance policies according to the classification of administrative units for commune, district, and provincial levels.
- Consistently implement the monthly allowance fund for non-specialized staff at the commune level, village, and neighborhood level based on the regular expenditure percentage of commune-level People's Committees; concurrently, regulate the maximum number of non-specialized staff according to each type of commune, village, and neighborhood level. On this basis, the commune-level People's Committees present to the People's Council at the same level to specifically regulate positions that receive allowances in the direction that one position can take on multiple tasks but must ensure the quality and efficiency of the assigned work.
The abolition of seniority allowances for officials when implementing wage reform from July 01, 2024. However, for grassroots healthcare public employees, according to current regulations, they are not subject to seniority allowances. Therefore, the abolition of seniority allowances as per Resolution 27 does not affect these subjects.
In addition, preventive healthcare public employees and grassroots healthcare public employees, according to current regulations, will be subject to preferential occupational allowances. Resolution 27 states that preferential occupational allowances, occupational responsibility allowances, and hazardous, dangerous allowances will be combined into occupational allowances.
Therefore, after the wage reform, preventive healthcare public employees and grassroots healthcare public employees will still enjoy this allowance.
What is the current preferential occupational allowance regime for preventive healthcare public employees?
Currently, Clause 7 Article 3 of Decree 56/2011/ND-CP supplemented by Article 1 of Decree 05/2023/ND-CP regulates the preferential occupational allowance policy from January 01, 2022, to December 31, 2023, for preventive healthcare public employees and grassroots healthcare public employees as follows:
7. The preferential occupational allowance level for preventive healthcare public employees and grassroots healthcare public employees applies from January 01, 2022, to December 31, 2023:
“a) The 100% allowance level applies to public employees who regularly and directly work in preventive healthcare (including border health quarantine); perform healthcare specialties at commune health stations, ward, commune-level town, regional polyclinics, maternity homes, district health centers, district, district-level town, provincial city, centrally-run city hospitals.
b) During the period of applying the preferential occupational allowance level for preventive healthcare public employees and grassroots healthcare public employees stipulated in this Clause 7, the provisions of Point c Clause 2 and the provisions for preventive healthcare public employees regularly and directly performing healthcare specialties in Clause 4 of Article 3 do not apply.
The preferential occupational allowance level for preventive healthcare public employees applies from January 01, 2022, to December 31, 2023, is 100%.
