4 Steps to Guide Administrative Procedures for Online Registration of Health Insurance Cards for Children Under 6 Years Old?
On June 12, 2023, Vietnam Social Security issued Decision 976/QD-BHXH in 2023 regarding the process of receiving and resolving administrative procedure dossiers by electronic interconnection: Birth registration, permanent residence registration, and issuance of health insurance cards for children under 6 years old on the National Public Service Portal in accordance with Decision 06/QD-TTg in 2022 of the Prime Minister of the Government of Vietnam.
According to Article 3 of the process issued with Decision 976/QD-BHXH in 2023, the process for receiving and resolving administrative procedure dossiers for birth registration, permanent residence registration, and issuance of health insurance cards for children under 6 years old includes 4 steps as follows:
Step 1. Citizens (Parents/ guardians/ relatives of children under 6 years old) must accurately and fully fill out the Interconnected Electronic Birth Registration, Permanent Residence Registration, and Health Insurance Card Issuance Declaration Form for children under 6 years old (Form No. 01 issued with Official Dispatch 2084/VPCP-KSTT in 2023).
Step 2. After the File Reception and Management System receives the electronic dossier from the Interconnected Public Service Software, the Social Security agency will:
- For complete and valid dossiers:
+ The staff at the Dossier Reception Department will transfer the dossier to the Revenue Management - Book, Card Department for processing and resolution.
+ The software system will automatically perform: Issuing a Social Security number and updating it in the Household Database as per the regulations in Decision 515/QD-BHXH in 2020; Updating revenue and usage value of the health insurance card into the designated management unit code as stipulated in Decision 595/QD-BHXH in 2017, Decision 505/QD-BHXH in 2020; Decision 490/QD-BHXH in 2023 and Decision 948/QD-BHXH in 2023 of the General Director of Vietnam Social Security (collectively referred to as the Decisions issuing the process for managing revenue, issuing Social Security books, and health insurance cards).
- For incomplete or invalid dossiers: Reject the dossier and clearly state the reason for rejection on the File Reception and Management System to notify the citizen within no more than 01 working day from the time of receiving the electronic dossier on the system.
- For invalid dossiers due to errors from the authority previously resolving the procedure (e.g., attaching the wrong electronic birth certificate,...), notify and clearly state the reason to that authority through the interconnected public service software for remediation as per the regulations.
Step 3. The Revenue - Book, Card staff at the Social Security agency will:
Daily, receive the dossier from the Dossier Reception Department, verify automatically generated data on the Revenue and Book Card Management Software, the detailed data table of Social Security, health insurance, and unemployment insurance contributions of participants (Form C69a-TS), the detailed allocation table of collected Social Security, health insurance, and unemployment insurance of participants (Form C83a-TS) for units with occurrences as per the Decision issuing the process for managing revenue, issuing Social Security books, and health insurance cards. Issue electronic health insurance cards (automatically returned to the personal data warehouse on the National Public Service Portal) or print physical health insurance cards as registered to transfer to the Dossier Reception staff.
Step 4. The staff at the Dossier Reception Department will receive data, physical health insurance cards from the Revenue - Book, Card Department to return results to the citizen as registered and conclude the health insurance card registration dossier on the File Reception and Management Software. The processing time for issuing health insurance cards for children under 6 years old is no more than 02 working days from the date of receiving the electronic birth certificate and electronic information and data from the Interconnected Public Service Software.

What does the birth registration, health insurance card issuance dossier for children under 6 years old include?
According to Article 7 of the Joint Circular 05/2015/TTLT-BTP-BCA-BYT, the dossier for interconnected administrative procedures for birth registration and health insurance card issuance for children under 6 years old includes the following documents:
- Birth registration declaration form as prescribed.
- Birth certificate issued by the medical facility where the child was born;
If the child was born outside a medical facility, the birth certificate shall be replaced by a witness's written confirmation; in the absence of a witness, the person registering the birth must make a sworn declaration about the actual birth.
In case of registration of birth for abandoned children, a report on the abandonment of the child shall replace the birth certificate.
In the case of birth registration for children born via surrogacy, an additional document proving the surrogacy as prescribed by law must be submitted.
- Health insurance participation declaration form as prescribed.
Where to submit the birth registration, health insurance card issuance dossier for children under 6 years old?
Submission of the birth registration, health insurance card issuance dossier for children under 6 years old is stipulated in Article 8 of the Joint Circular 05/2015/TTLT-BTP-BCA-BYT:
- Individuals requesting birth registration and health insurance card issuance for children under 6 years old must submit 01 set of documents to the People's Committee at the commune level.
- Individuals requesting birth registration and health insurance card issuance for children who are unable to directly submit the documents to the commune-level People's Committee may authorize another person to do so on their behalf. The authorization must comply with current legal regulations.
Decision 976/QD-BHXH in 2023 takes effect from July 01, 2023.
