(1) Detailed Information of the Communist CPV member:

Full name; date of birth; position within the Communist Party and government or organizational role (if any), along with the working unit and the name of the Party branch.

(2) Strengths and Achievements

First, the CPV member should highlight the strengths acquired during the past year. This should comprehensively cover the following aspects:

- Regarding political qualities, ethics, lifestyle; awareness of organizational discipline; behavior, and working style:

+ Self-assess according to specific levels

+ It can be filled out as follows:

(1) Absolute loyalty to Marxism-Leninism, Ho Chi Minh's thoughts, and the Communist Party's reform path; adhering to, promoting, and encouraging family to observe the Party's policies and governmental law...

(2) Complying with organizational assignments; rigorously following what members of Communist Party of Vietnam must not do; fully participating in meetings and paying party fees; setting an example in fulfilling civic duties and maintaining regular contact with the branch committee and grassroots Party committee at the place of residence...

- The execution of tasks, authority, and results achieved regarding assigned targets and duties throughout the year.

+ Self-assess according to specific levels

+ It can be filled out as follows:

The results of fulfilling assigned targets and duties throughout the year should be quantified by products. For CPV members who are officials and public employees, they should clarify the volume, quality, progress, efficiency implemented of the assigned targets and duties throughout the year; spirit of innovation, creativity, self-responsibility; awareness, and attitude in serving the public...

- The evaluation and classification of organizations, agencies, and units under the direct leadership and management scope; periodic trust evaluation results (if any).

- The results of leadership, direction, management operation; execution of responsibility, duties; the degree of task completion of organizations, agencies, and units under management; ability to rally and build internal unity.

+ Self-assess according to specific levels

+ It can be filled out as follows:

Thoroughly grasp, institutionalize, and execute the Party's policies, governmental laws in agencies, organizations, units;

Maintain discipline and order in agencies, organizations, units; prevent incidents and violations of discipline or law warranting handling, avoid prolonged complaints and denunciations; anti-corruption, waste prevention within the scope of agencies, organizations, units;

- Responsibility in work; energetic, innovative, creative, courageous attitude in thinking, acting, and responsibility; handle complex, sensitive issues when executing tasks.

+ Self-assess according to specific levels:

+ Subsequently, it can be referred to the following criteria for filling:

- Emphasize responsibility, dedication to work; lead and manage units to excel in assigned tasks; actively study, research, acquire, and apply effectively modern scientific knowledge, technologies, and initiatives in work and production.

- Be proactive, innovative, dare to think, act, and take responsibility; align words with actions, fulfill commitments.

- Understand and properly execute individual responsibilities, duties, and rights; work with principles, discipline, rationality, and empathy; avoid abusing position, authority when resolving work; combat against manipulation, influence peddling, crime evasion, qualification manipulation, accolade pursuit, and other negative behaviors.

- Resolve against local, factional mentality and opportunistic, practical behaviors for personal gain, "group interests."

- Set personal and family exemplary responsibility; combat against corruption, misconduct, wastefulness; earn trust from officers and CPV members.

Afterward, you can refer to the following criteria for filling:

+ Actively participate in training and personal capacity development according to Directive 05-CT/TW 2015 on enhancing the study and adherence to Ho Chi Minh’s thoughts, ethics, style, and rules on exemplary responsibility. Thereby working more effectively in one’s role, aptly applying professional knowledge and skills in daily tasks;

+ Shift thinking, apply new knowledge and skills they learned to work, create innovative ideas, solutions to solve problems.

(5) Explanation for Issues Suggested for Review (If Any)

Explain each issue suggested for review, cite causes, and determine personal responsibility for each suggested issue.

Example:

Issue: Over the past year, I have not prioritized self-study and professional enhancement, leading to limitations in grasping and applying new technology in work.

Cause: Due to over-focusing on current work and lacking specific plans for self-study, I have not allotted enough time for updating knowledge. Furthermore, I am not yet acquainted with using new materials and learning resources.

Remedial Measure: I have planned weekly self-study sessions and registered for online courses related to the field of work. Additionally, I have sought guidance from more experienced colleagues to improve my skills. I am committed to continuous learning and enhancing my specialization.

Progress Commitment: I am well aware of my limitations and am committed to addressing them in the time ahead. I earnestly hope to receive support and feedback from colleagues to improve myself and fulfill tasks successfully.

(6) Clarifying Personal Responsibility for the Limitations, Shortcomings of the Collective (If Any)

Example:

Shortcomings of the collective: In the past year, the collective has not well-executed cooperative and supportive efforts among individuals, leading to certain tasks not meeting required progress and quality.

Personal Responsibility: As a group member, I have not proactively suggested meetings for information exchange or supported colleagues when they faced difficulties. I also lacked tight coordination in sharing documents and knowledge related to common tasks.

Remedial Measure: I am committed to being more proactive in proposing periodic internal meetings for information exchange, swiftly supporting colleagues experiencing difficulties, and sharing important documents for collective reference.

Responsibility Spirit: I fully recognize my responsibility in this shortcoming and am committed to improving to aid effective cooperation within the collective.

(7) Direction and Measures to Remediate Limitations, Shortcomings Reference the following criteria for filling:

+ Not daring to express personal opinions at meetings.

+ Personally not sensitive in social relationships.

+ Personally not daring in self-assessment and criticism work. Not daring to challenge.

(8) Self-assessed Quality Rating Level

This is a significant part of the self-assessment form. The rating levels include Outstanding task completion; Good task completion; Task completion; Task non-completion.

(9) Sign and Clearly State the Name of the Communist CPV member Completing the Assessment

(10) Feedback from Agency Head, Branch Committee (Cell), Party Committee

After the CPV member self-assesses their rating, based on information presented in the self-assessment form submitted by the CPV member at the year-end evaluation, levels including the head of the agency, unit; branch committee, and Party committee will evaluate that CPV member.

- The agency, unit head of that CPV member: Provide comments, evaluation, and suggest a quality rating level (if officials and public employees).

- Branch Committee: Summarize the self-rating level of the CPV member, incorporate feedback from competent agencies where the CPV member resides to propose a rating level.

- Party Committee: Summarize, appraise, based on the proposal from the branch committee to determine the quality rating level for the CPV member.

How much are membership fees in economic organizations in Vietnam?

Based on Subitem 3 Item 1 Section B of the Regulation on Party fees issued with Decision 342-QD/TW 2010, the subjects and monthly fee rates for CPV members are specified as follows:

Subjects and Monthly Party Fee Rates for CPV members

Monthly income of CPV members for calculating party fees includes: salary, certain allowances; wages; living expenses; other income. CPV members who can ascertain their regular income shall pay Party fees as a percentage (%) of the monthly income (before personal income tax deduction); if the income is not ascertainable, specific monthly rates are set for different types of subjects.

3- CPV members working in enterprises, public service providers, and economic organizations: pay party fees monthly by 1% of the salary, wages, and other income from the unit's salary fund.

Thus, as per regulations, CPV members working in economic organizations shall pay monthly Party fees by 1% of the salary, wages, and other income from the unit’s salary fund.
