What are the instructions for filling out the notice form of application receipt and appointment to return the results of issuing health insurance cards in Vietnam?
Pursuant to the instructions in Form No. 4 of the Appendix to Administrative Procedures issued together with Decision 62/QD-BYT in 2023, the instructions for filling out the notice form of application receipt and appointment to return the results of issuing health insurance cards in Vietnam in Vietnam are as follows:
(1) The notice of application receipt and appointment to return the results shall be prepared in 02 copies, including 1 copy provided for the applicant, and 1 copy handed over with the application to the professional department to be stored at the department receiving and returning the results.
(2) Application receipt and appointment to return results are made for each type of application according to each administrative procedure (for example, a unit submitting 3 different types of applications will be provided 3 notices of application receipt and appointment to return the results).
Accordingly, a notice of application receipt and appointment to return the results includes the following basic contents:
- Applicant;
- Name of the unit (if it is a representative of the applicant); Unit code;
- Full name of the health insurance participant;
- Health insurance card code;
- Initial healthcare establishment;
- Address;
- Contact phone number;
- Email (if any);
- Requests to be settled;
- Composition of submitted application (Name of the application, quantity, notes)
- Time limit for processing application;
- Time to receive applications;
- Time to return the results of processing application;
- Register to receive results at:
- Department of receiving applications and returning results
- Via postal service (with the Address to receive the results)
Money from settling health insurance benefits shall pay at:
- Social Insurance Agency
- Via personal account (with Account Number; Bank; Account holder's name)
(3) In the request section: Write a summary of requests to be settled administrative procedures.
What are the required documents when receiving the settlement results of health insurance benefits in Vietnam?
Pursuant to the instructions in Form No. 4 of the Appendix to Administrative Procedures issued together with Decision 62/QD-BYT in 2023, individuals who register to receive results directly at the social insurance agency, the officials receiving the application and returning the results shall guide as follows:
a) The beneficiary: provides appointment notes and citizen ID cards or ID cards.
b) The authorized person:
- In case of a relative of the beneficiary: Provide the following documents:
+ Appointment note;
+ ID card or citizen ID card:
+ One of the following documents proving that he is a relative of the health insurance beneficiary: a copy of the birth certificate or birth certificate or marriage certificate or a certificate of residence information or a notice of personal identification number and citizen information in the national database on population.
- In case of a guardian: Provide the following documents:
+ Appointment note;
+ ID card or citizen ID card;
+ Proof of natural guardianship of the beneficiary (copy of birth certificate or birth certificate or marriage certificate or certificate of residence information or Notice of personal identification number and citizen information in the national database on population).
+ In case there is no natural guardian as prescribed by law, provide an appointment certificate, identity card, or citizen identity card, and the decision to recognize the guardianship of the competent authority.
- In case of a relative or guardian: provide appointment note, ID card or citizen ID card, and power of attorney in accordance with current laws.
Thus, when receiving the settlement results of health insurance benefits in Vietnam, individuals must present the above documents.
