According to the Appendix issued with Decision 896/QD-BHXH 2021 on the List of online public services for administrative procedures as follows:
- Implementing agency: Provincial/district social insurance In Vietnam
- Objects of implementation: Organization
- Applicable cases:
+ The unit registers to participate for the first time;
+ The unit moves from another location to another;
+ Unit changes information.
- Profile composition
For employees:
+ Declaration of participation and adjustment of information on social insurance and health insurance (Form TK1-TS);
In case the employee has been granted a social insurance number, the Form TK1-TS is not required.
+ In case the employee is entitled to higher health insurance benefits: additional proofs (if any) according to Appendix 03 issued together with Decision No. 595/QD-BHXH dated 14/4/2017 (amended in Decision No. 595/QD-BHXH). 505/QD-BHXH dated March 27, 2020).
For employers:
+ Declaration of participating units, adjustment of information on social insurance and health insurance in Vietnam (Form TK3-TS);
+ Report on employment situation and list of participants in social insurance and health insurance in Vietnam and unemployment insurance (Form D02-LT);
+ List of information (Form D01-TS).

What is the order and method of registration and change of payment information of social insurance, health insurance, unemployment insurance, occupational accident and occupational disease insurance?
According to the Appendix issued with Decision 896/QD-BHXH 2021 on the List of online public services for administrative procedures as follows:
(1) Execution sequence
Step 1: Prepare and submit a dossier to the social insurance agency
Employer registering for the first time, moving from another location to
(a) Employee: Prepare Declaration Account TK1-TS.
(b) Unit:
- Guide employees to make TK1-TS declaration;
- On the basis of the declaration TK1-TS of each employee, the unit shall make Form D02-LT;
- Prepare TK3-TS declaration;
- Prepare Form D01-TS.
- The employer changes the closing information: Make a declaration TK3-TS.
Step 2. The social insurance agency receives the application.
Step 3. The unit receiving the results has been resolved.
(2) How to do it
Step 1. Submit application: The employer chooses to submit the application in one of the following forms:
- Make an electronic file, digitally sign it and send it to the Public Service Portal of the Vietnam Social Security Administration or through the I-VAN organization or through the National Public Service Portal;
- Via Postal Service;
- Submit directly at the dossier-receiving department of the provincial and district social insurance agencies or at the HCC Service Centers at all levels.
Step 2. Receive settlement results according to the registered forms.
- Time limit for settlement: No more than 05 days from the date of receipt of complete dossiers as prescribed.
