(1) Detailed information about the Communist Party member:

Name; date of birth; position in the Communist Party and position in authorities or organizations (if any) along with the name of the affiliated cell.

(2) Strengths and achievements

- Strengths in political ideology

+ Discuss the adherence to political ideology, loyalty to the Communist Party, and strict compliance with its regulations and state laws.

+ Clearly state the determination to build and protect the Party organization and contribute to building a clean and strong Party.

Example:

"Always uphold political ideals, absolutely loyal to the Communist Party, adhere well to the policies of the Party and the State. Actively participate in political activities organized by the affiliated cell, contributing to fostering solidarity and responsibility within the organization."

- Achievements in professional tasks and assigned duties

+ Evaluate the completion of tasks, work results, and any valuable initiatives or contributions during work.

+ Clearly state efforts and creativity in work, specific results, and achieved efficiency.

Example:

"Excelled in completing assigned professional tasks, always proactive and creative at work. Proposed and successfully implemented several improvement solutions that enhanced operational efficiency, earning high evaluations from the unit and colleagues."

- Strengths in ethical qualities and lifestyle

+ Evaluate morality, a clean and exemplary lifestyle. Clearly state commitment to fighting corruption, negativities, and compliance with Party regulations on ethical standards for Party members.

+ Emphasize exemplary conduct within the collective, spirit of solidarity, assisting comrades, and the public.

Example:

"Always maintain ethical qualities, live honestly, with integrity, and modesty. Exemplary in work and life, ready to help colleagues, actively participate in building a cohesive and united collective."

- Achievement in political theory education and professional training

+ Present participation in all training programs on political theory and professional skills organized by the Party and unit.

+ Clearly state improvements in professional capacity, self-research to enhance knowledge.

Example:

"Participated fully in political and professional development programs, continuously self-study to enhance knowledge for better serving work. Always update new directives and instructions to execute tasks precisely and effectively."

- Participation results in movements, social activities, and Party building

+ Specifically list movements and activities you have participated in and contributed to. Emphasize active and exemplary participation in collective activities, social movements organized by the affiliated cell and unit.

+ If there are specific achievements, clearly state them to showcase personal involvement.

Example:

"Actively participate in competitive movements and charitable, social activities launched by the unit and locality, contributing to enhancing the image of Party members in the community. Recognized as a model Party member in collective activities by the affiliated cell and comrades."

(3) Limitations, shortcomings, root causes

Alongside achievements, limitations and shortcomings accompany them. In this self-assessment, Party members must point out their shortcomings from the year and their root causes. Refer to the following criteria for writing:

- Limitations in political ideology

+ Evaluate deficiencies in studying and implementing Ho Chi Minh's ideology, ethics, and style if any.

+ Comments on inadequate expressions like lack of proactivity in updating information or occasional inactivity in participating in political activities or Party theory studies.

Example:

"At times, lack of proactivity in updating new political information, infrequent participation in political theory sessions, and need for enhancing study of Ho Chi Minh's ideology and ethics."

- Limitations in task execution

+ Present work limitations, such as not achieving maximum effectiveness in certain tasks, delays, or not reaching set goals.

+ Comments on lack of proactivity or flexibility in handling work.

Example:

"Some tasks didn't achieve high effectiveness due to inefficient time management. Occasionally, not proactive in grasping and resolving issues, leading to delays in several tasks."

- Limitations in study, professional development, and self-training

+ Present deficiencies or delays in professional and political theory development, not dedicating sufficient time for self-study and self-training.

+ Clearly point out areas needing improvement in professional knowledge or workplace skills.

Example:

"Self-study and political theory development were not carried out regularly. Occasionally, insufficient time devoted to professional enhancement, leading to less effective work application."

- Causes of limitations and shortcomings

+ Present subjective and objective causes leading to the above limitations. Note to point out specific and clear reasons why some tasks were not well-completed.

+ Causes may include personal skills needing improvement or objective conditions like a large workload, insufficient guidance.

Example:

"The main cause is the limited personal time management and work organization skills. Additionally, the large workload and handling multiple tasks at once sometimes prevent fulfilling all work requirements well."

(4) Results of overcoming limitations and shortcomings pointed out in previous assessments

Assess clearly each limitation and shortcoming (resolved; being resolved, resolution level; unresolved); difficulties, challenges (if any); and individual responsibility.

Party members self-assess their performance level: Choose one of the levels (Excellent; Good; Average; Poor)

(5) Explanation on issues suggested for assessment (if any)

Explain each issue suggested for assessment, state the cause, and identify individual responsibility for each issue suggested for assessment.

(6) Clarify individual responsibility with collective limitations and shortcomings (if any)

- Political ideology corrective measures

+ Propose measures to enhance awareness, strengthen study, and practice Ho Chi Minh's ideology, ethics, and style.

+ Set plans to attend all political meetings and regularly update Party directives and resolutions.

Example:

"Commit to allocating time for studying and updating new Party directives and resolutions. Attend all political meetings organized by the affiliated cell, while proactively learning to strengthen ideological standing and enhance political awareness."

- Professional task correction measures

+ Propose measures to improve work skills, enhance proactivity, and flexibility in work.

+ Clearly outline plans for reasonable time management, learning new skills, and applying work management tools if needed.

Example:

"Proactively learn additional work management and time organization skills. Commit to improving problem-solving skills and proactivity, ensuring tasks are completed on time and at high quality."

- Study and professional training corrective measures

+ Plan for political theory and professional study, participate fully in training programs required by the organization.

+ Commit to self-study and additional research outside of working hours to enhance proficiency and better meet work demands.

Example:

"Commit to participating in all political and professional development programs, proactively self-study and research to improve professional competency. Dedicate time outside working hours to training and updating knowledge to enhance work effectiveness."

(8) Self-assessment quality ranking

This is an essential content in the self-assessment. The ranking levels include: Excellent completion of duties; Good completion of duties; Completion of duties; Non-completion of duties.

(9) Sign and clearly state the name of the Party member conducting the assessment

After the Party member self-assesses, based on the information provided in their year-end Communist Party self-assessment, the hierarchies consisting of the head of the agency, unit; branch and Party committee will evaluate that Party member.

+ Not having spent time to explore and study the Communist Party’s platform and charter, the state’s policies and laws.

+ Not being proactive in proposing and advising on solutions to perform better in professional work.

+ Not bold in contributing opinions in party meetings; showing favoritism in criticism and self-criticism efforts…

- Causes of Limitations and Deficiencies.

You can rely on the following criteria for filling out:

+ Lack of personal motivation and the importance of self-awareness in personal development.

+ Delayed in updating information and knowledge.

+ Lack of personal discipline and responsibility….

+ Favoritism causing evaluation and criticism work to not be truly objective, failing to assess the true nature of issues.

(3) Results of Overcoming Previously Identified Limitations and Deficiencies Concluded or Pointed out by Authoritative Levels in Previous Review Periods

+ Actively participate in training and personal capacity development according to Directive 05-CT/TW 2015 on promoting study and following Ho Chi Minh’s thoughts, ethics, style and regulations on the responsibility to set an example. This leads to more effective performance in their roles, appropriately applying specialized knowledge, skills into daily work;

+ Change in mindset, having applied new knowledge and skills learned into work, creating innovative ideas, solutions for problem-solving.

(4) Explanation of Suggested Review Issues (if any)

Example: Deficiencies in Coordination Work

Issue: Over the past year, I have not effectively coordinated with comrades within the unit, leading to some tasks being delayed and affecting overall efficiency.

Cause: The main cause was work pressure during peak periods, coupled with my inefficiency in organizing work. In addition, I lacked initiative in regularly engaging with comrades for better cooperation.

Remedial Action: I have been developing more specific work plans, sharing duties with colleagues, and establishing a more frequent schedule of exchanges within the unit. Additionally, I will strive to enhance my time management skills to complete tasks on time.

Spirit of Improvement: I sincerely apologize for this shortcoming and will profoundly learn from it. I commit to improving myself for better cooperation with comrades and colleagues in the future.

(5) Clarify Personal Responsibility for Collective Limitations (if any)

Example: Shortcomings in Coordination and Mutual Support

Collective Deficiency: Over the past year, the collective has not sufficiently coordinated and supported individuals, causing some tasks not to meet progress and quality requirements.

Personal Responsibility: As a group member, I have not actively proposed meetings to exchange information or support comrades in difficulties. I also lacked close coordination in sharing documents and knowledge related to common work.

Remedial Action: I commit to being more proactive in proposing regular internal meetings to exchange information, promptly supporting comrades in difficulties, and sharing important documents for common reference.

Sense of Responsibility: I am fully aware of my responsibility in this deficiency and commit to making improvements to help the collective coordinate more effectively.

(6) Directions and Measures to Overcome Limitations and Deficiencies

You can use the following criteria for filling out:

+ Not being courageous enough to express personal opinions in meetings.

+ Not being sensitive enough in social relationships.

+ Not brave enough in self-criticism and criticism work.

+ Not bold in fighting.

(7) Self-Assessment of Quality Classification

This is one of the important contents of the personal review. The classification levels include Outstanding Completion; Good Completion; Completion; and Non-Completion of Duties.

After the Communist Party member self-assesses their classification level, based on the information given by the Communist Party member in the year-end Communist Party member self-assessment, heads of the agency, unit; party cell, and Communist Party Committee will evaluate this Communist Party member.

- Head of the agency, unit of that Communist Party member: Comments, evaluates, and issues a quality classification (if they are officials and public employees).

- Party Cell: Aggregates the self-classification level of the Communist Party member, opinions from competent authorities where the Communist Party member resides to propose a classification level.

- Committee: Aggregates, appraises, and bases on the proposal of the party cell to deliver a Communist Party member quality classification.
