The current health insurance card code structure is specified in Decision 1351/QD-BHXH dated November 16, 2015, specifically as follows:
The health insurance card code consists of 15 characters, divided into four boxes.

Is denoted by letters (according to the Latin alphabet) and is the code of the participants in health insurance. In cases where a person belongs to many different beneficiaries of health insurance, the object code written on the health insurance card is the code of the person paying the health insurance premium, determined first according to the provisions of Clause 7, Article 1 of the Law amending and supplementing the Law on Health Insurance.
- Enterprise: Employees working in enterprises established and operating under the Law on Enterprises and the Law on Investment.
- HX: Employees working in cooperatives and unions of cooperatives established and operating under the Law on Cooperatives.
- CH: Employees working in state agencies, non-business units, the armed forces, political organizations, socio-political organizations, socio-political-professional organizations, and other social organizations.
- NN: Employees working in foreign agencies, organizations, or international organizations in Vietnam, unless otherwise provided for by an international treaty to which the Socialist Republic of Vietnam is a signatory.
-TK: Employees working in other employers' organizations established and operating in accordance with law.
- HC: Cadres, civil servants, and public employees in accordance with the law on cadres, civil servants, and public employees.
- Export: Persons working part-time in communes, wards, and townships in accordance with the law on cadres and civil servants;
- HT: Persons receiving monthly pensions and benefits for loss of working capacity;

- CS: Retired rubber workers are enjoying monthly allowances according to Decision 206/CP dated May 30, 1979, of the Government Council (now the Government) on policies for newly liberated workers who do heavy and unhealthy jobs who are now old and weak and have to quit their jobs;
- QN: Officer, professional soldier, non-commissioned officer, soldier of the Vietnam People's Army in active service; people doing cipher work are paid the same salary as military personnel working at the Government Cipher Department; cipher students enjoy living expenses from the State budget according to the same regimes and policies as military students;

- Education: People participating in household health insurance include people in the household, except for the subjects specified in the above items.
Signed by a number (in the order of 1 to 5) is the level of health insurance benefits. In cases where a person belongs to many subjects participating in health insurance, the level of health insurance benefit stated on the health insurance card is the benefit level of the person with the highest benefits.

Signed by a number (from 01 to 99) is the code of the province or city directly under the Central Government, where the health insurance card is issued (according to the code of the province and city issued together with Decision 124/2004/QD-TTg dated 08/7/2004 of the Prime Minister on the promulgation of the list and codes of administrative units of Vietnam and Official Dispatch628/TCTK-PPCD dated August 6, 2009 of the General Statistics Office on the announcement of the new administrative list code).
Particularly, the code of the Social Security of the Ministry of Defense is denoted by number 97, of the Social Security of the People's Public Security is denoted by number 98.
