In Article 2 of Decree 146/2018/ND-CP, the structure of the health insurance card code is as follows:
The first two characters (first box): denoted by letters (according to the Latin alphabet), is the code of subjects participating in health insurance.
In case a person belongs to many different beneficiaries of health insurance, then:
The subject code written on the health insurance card is the code of the person paying health insurance premiums determined first according to the provisions of Clause 7, Article 1 of the Law amending the Law on Health Insurance.
*Groups paid by employees and employers:
- DN: Employees of enterprises established and operating under the Law on Enterprises and Law on Investment.
- HX: Employees of cooperatives and cooperative unions established and operating under the Law on Cooperatives.
- CH: Employees working in state agencies, non-business units, armed forces, political organizations, socio-political organizations, socio-political-professional organizations, organizations socio-professional organizations and other social organizations.
- NN: Employees working in foreign agencies, organizations or international organizations in Vietnam, unless an international treaty to which the Socialist Republic of Vietnam is a contracting party contains other provisions.
- TK: Employees working in other employers' organizations established and operating in accordance with law.
- HC: Cadres, civil servants, and public employees in accordance with the law on cadres, civil servants, and public employees.
- Export: Persons working part-time in communes, wards, and townships in accordance with the law on cadres and civil servants;
*Groups paid by the social insurance organization
- HT: Persons receiving monthly pensions or benefits for loss of working capacity;
- TB: People who are enjoying a monthly social insurance allowance due to occupational accidents or occupational diseases;
- NO: The employee who is absent from work is entitled to sick leave under social insurance law because of an illness on the Minister of Health's list of diseases requiring long-term treatment.
- CT: Persons aged full 80 years or older who are receiving monthly survivorship allowance;
- Publisher: Commune, ward, and township officials who have quit their jobs are enjoying monthly social insurance allowances;
- TN: The person who is receiving unemployment benefits according to the provisions of the law on unemployment insurance;
- CS: Retired rubber workers are enjoying a monthly allowance according to Decision No. 206/CP dated May 30, 1979 of the Government Council (now the Government) on policies for newly liberated workers who do heavy and unhealthy jobs, who are now old and weak and have to quit their jobs;*The group paid by the State budget
- QN: Officer, professional soldier, non-commissioned officer, and soldier of the Vietnam People's Army in active service; people doing cipher work are paid the same as military personnel working at the Government Cipher Department; cipher students receive state budget living expenses under the same regimes and policies as military students;
- CA: Officers, professional non-commissioned officers and officers, professional and technical non-commissioned officers, non-commissioned officers and conscription soldiers working in the people's police force; People's Public Security trainees enjoy living expenses from the State budget;
- CY: People doing cipher work receive a salary like military personnel working in cipher organizations under ministries, branches, and localities;
- XN: Commune, ward, and township officials who have quit their jobs are receiving monthly allowances from the State budget;
- MS: The person who has stopped enjoying the allowance for loss of working capacity is receiving a monthly allowance from the State budget;
- CC: People with meritorious services to the revolution, including: People who were active in the revolution before January 1, 1945; revolutionary activists from January 1, 1945 to the August 1945 uprising; a heroic Vietnamese mother; war invalids and beneficiaries of policies such as war invalids, class B war invalids, and sick soldiers with a working capacity decrease of 81% or more;
- CK: People with meritorious services to the revolution according to the law on people with meritorious services to the revolution, except for those who are granted CC codes;
- CB: Veterans according to the law on veterans;

- BT: People who are legally entitled to a monthly social protection allowance.
- HN: People from low-income families;
- DT: Ethnic minorities are living in areas with difficult or extremely difficult socio-economic conditions;
- DK: People who live in extremely difficult socioeconomic conditions;
- XD: Residents of island communes and districts;
- TS: Relatives of people who have made meritorious contributions to the revolution include biological fathers, natural mothers, spouses, and children of martyrs; people who have made meritorious contributions to the raising of martyrs.
- TC: Relatives of people with meritorious services to the revolution, including:
Biological parents, spouses, and children from the age of six to under the age of eighteen, or from the age of eighteen if they continue to attend school or have severe disabilities, particularly severe disabilities in the following subjects: revolutionary action before January 1, 1945;
People who participated in the revolution between January 1, 1945, and the day of the August uprising in 1945;

People serving resistance activists infected with toxic chemicals with reduced working capacity from 81% or more live at home.

- CN: People from near-poor households;
- HS: Students who are studying at educational and training institutions of the national education system;
- SV: Students who are studying at educational and training institutions, vocational training institutions of the national education system;
- GB: People of households working in agriculture, forestry, fishery and salt production have an average standard of living as prescribed by law;

GD: Participants in household health insurance include those in the household, except for those specified at points a, b, c, d above.
Signed by a number (in the order from 1 to 5) is the level of health insurance benefits.
In cases where a person belongs to many subjects participating in health insurance, the level of benefit stated on the health insurance card is the benefit level of the person with the highest benefits.
- Symbol with number 1: The health insurance fund pays 100% of medical examination and treatment costs covered by health insurance coverage; does not apply the Minister of Health's limit on payment rates for a number of drugs, chemicals, medical supplies, and technical services on the list and rates and conditions for payment of technical services;
Expenses for transporting patients from district level to a higher level in case of emergency or during inpatient treatment requiring professional and technical transfer, including beneficiaries with symbols: CC, TE.
- Symbol with the number 2: The health insurance fund covers 100% of medical expenses (with limited payment rates for some drugs, chemicals, medical supplies, and technical services as prescribed by the Minister of Health);
Expenses for transporting patients from the district level to a higher level in the event of an emergency or during inpatient treatment that requires professional and technical transfer, including beneficiaries with the symbols: CK, CB, KC, HN, DT, DK, XD, BT, TS.
- The health insurance fund pays 95% of medical expenses within the scope of health insurance coverage (payment rates for some drugs, chemicals, medical supplies, and technical services are limited as prescribed by the Minister of Health);
At the commune level, 100% of medical care costs are covered, and the cost of a single medical treatment is less than 15% of the basic monthly salary, including beneficiaries with the symbols HT, TC, and CN.
- Symbol by number 4: the fund covers 80% of medical expenses covered by the health insurance fund (payment rates for some drugs, chemicals, medical supplies, and technical services are limited as prescribed by the Minister of Health);
100% of medical treatment costs at the commune level and the cost for one time of medical treatment is lower than 15% of the basic monthly salary, including beneficiaries with symbols as: DN, HX, CH, NN, TK, HC, export, TB , NO, CT, XB, TN, CS, XN, MS, HD, TQ, TA, TY, HG, LS, PV, HS, SV, GB, GD.
- Symbol with number 5: 100% of medical expenses paid by the health insurance fund, including medical expenses outside the scope of health insurance coverage; shipping costs, including beneficiaries with symbols QN, CA, CY.
Next two characters (3rd cell):
Signed by a number (from 01 to 99) is the code of the province or city directly under the Central Government, where the health insurance card is issued (According to the provincial and city codes issued in conjunction with Decision 124/2004/QD-TTg on the promulgation of the list and codes of administrative units in Vietnam and Official Dispatch 628/TCTK-PPCD 2009 on the announcement of the new administrative list code).
The code of the Social Insurance Ministry of National Defense is denoted by number 97, and the code of the People's Public Security Social Security is denoted by number 98.
The last ten characters (the fourth box) are the personal identification numbers of the participants in social insurance and health insurance as prescribed in Decision 1263/QD-BHXH 2014 and regulations on the management and use of the system of shared lists and reporting norms for the social insurance sector.
