On March 18, 2024, the Vietnam Minister of National Defense issued Circular 13/2024/TT-BQP amending the legislative documents under the authority of the Minister of National Defense related to the declaration of personal information when carrying out administrative procedures.
The medical record is a legal basis for retired officers to receive medical services at military hospitals in Vietnam.
The medical record is printed according to Form No. 01/2024/KB-SQPV issued with Circular 13/2024/TT-BQP. To be specific:
- The record is printed in A5 size, consisting of 34 pages (including the cover);
- Regulations on printing cover pages;
+ Cover page 1: Printing the name of the medical record for retired officers;
+ Cover page 34: Printing the regulations on the use of the medical record for retired officers.
- Regulations on printing inner pages (from pages 3 to 32);
+ Page 3: Printing the résumé section;
+ From page 4 to page 32, printing the page numbers with columns for hospitals to write when retired officers undergo medical examination and treatment.
- The medical record issued to retired officers will be stamped by the hospital at the junction between the pages.
Management and use of the medical record are regulated in Article 16 of the provisions issued with Decision 33/2003/QD-BQP (amended by Circular 139/2011/TT-BQP) as follows:
- The medical record is printed and issued by the military hospital, charged at cost at the time of printing.
- The medical record can be used for multiple examinations; when the record runs out of space or is lost or damaged, the retired officer will be issued a new record by the hospital where they are registered for medical examination and treatment.
- Between examinations, retired officers are responsible for managing the record to avoid loss or damage.
