The basic content of a form of certification of work includes information such as the employee's full name, telephone number, ID card number, residential address, the name of the organization, the duration of employment, the reason for requesting the letter, and the confirming signatures and official seal of the organization.
Below are guidelines for filling out a form of certification of work:
(1) This is the department responsible for receiving and approving the request. Depending on the organization where you work, the head of the organization can be the Director, the Unit Manager, or a relevant authority directly supervising your work (e.g., the Human Resources Manager of the company).
(2) Fill in your personal information accurately according to your ID card or citizen ID card, along with your permanent address and current residence.
(3) Provide detailed information about the company where you have been employed, including the full name and address of the company. In particular, pay special attention to providing the telephone number of the direct supervisor or the person responsible for overseeing your work so that the organization or institution that requires your certification of work to verify the accuracy of the information can contact the department or unit where you have been employed to confirm the authenticity of your work record.
(4) Clearly state the positions and job titles you have held during specific periods if you have changed positions or departments. The more details, the better.
(5) Specify the department or section where you have worked to give an overview of the tasks you have been involved in. For example, Marketing Department, Human Resources Department, Administrative and Accounting Department, etc.
Regarding the type of employment contract, mark whether it is a fixed-term contract of 1 year, 2 years, or 3 years, or an indefinite-term contract, depending on the type of contract you have signed with the company. Other types of contracts may include short-term contracts of less than 3 months, such as internships or probationary contracts.
(6) Clearly state the reason for requesting the form of certification of work according to your needs. It could be for visa applications, obtaining a Mastercard, preparing a study abroad application, or verifying work experience. This will help the approving department trust you and provide more effective support in confirming your employment.
The confirmation section must include the complete signatures and stamps of the approving department, as well as the signature of the person writing the letter.
