Application for consideration for awarding the title of "Doctor of Merit" according to the regulations stated in Article 14 of Decree 25/2024/ND-CP (effective from April 15, 2024), includes:
- A report on the achievements of the individual proposing the title of "Doctor of Merit" using Form No. 09 as prescribed in the Appendix issued along with Decree 25/2024/ND-CP.
- A certified copy of the decision to award commendations or rewards related to the proposed title.
- A certified copy of the certificate or decision from the competent management authority regarding the innovation or exclusive patent (if any).
- A certified copy of the report on the assessment and evaluation of scientific and technological tasks, the decision to recognize the results of scientific and technological tasks with a rating of at least "achieved" or above, and a certification from the competent authority on the effectiveness and impact of the scientific and technological tasks.
- Confirmation of the duration of direct professional and technical work in the medical field. In cases where the healthcare facility has been dissolved or merged, the immediate superior of that unit must confirm the duration of professional and technical work in the medical field.

Therefore, the application for consideration for awarding the title of "Doctor of Merit" includes:
- A report on achievements according to the specified form.
- A certified copy of the decision to award commendations or rewards.
- A certified copy of the certificate for innovation or patent (if any).
- A certified copy of the report on the assessment of scientific and technological tasks.
- Confirmation of the duration of professional work in the medical field.

The procedure for awarding the title of "Doctor of Merit" consists of the following 4 steps:
Step 1: The individual proposing the title of "Doctor of Merit" shall prepare or authorize another individual or organization to prepare the application file according to the regulations stated in Article 14, Article 2 of Decree 25/2024/ND-CP. The application file shall be submitted in paper format (original copy) and accompanied by electronic files or submitted online or through postal services to the grassroots-level Council (through the Standing Committee of the Council).
Step 2: Procedure for awarding at the grassroots-level Council
Step 3: Procedure for awarding at the provincial or ministerial-level Council
Step 4: Procedure for awarding at the State-level Council
- The State-level Council receives the application file from the provincial or ministerial-level Council.
- The Standing Committee of the State-level Council receives and reviews the composition and validity of the documents in the application file submitted by the provincial or ministerial-level Council, reviews and reports to the competent authority on any related recommendations regarding the awarding of the title (if any), sends the documents as prescribed in points d, e, g, paragraph 6 of Article 14 of Decree 25/2024/ND-CP to the members of the Council, and organizes Council meetings.
- The State-level Council performs the following tasks:
+ Holds a closed meeting and votes to select individuals who meet the qualifications and criteria for the title of "Doctor of Merit". Individuals who receive at least 90% of the total votes from the Council members, as stipulated in the decision to establish the Council, will be included in the list submitted to the Prime Minister for consideration and decision by the President.
+ Announces the results of the selection process publicly on the Government's electronic information portal, the Ministry of Health's electronic information portal, and the Health and Life newspaper for a period of 10 days.
+ In the case of any recommendations or complaints, the Council is responsible for reviewing and reporting them to the competent authority for handling according to the provisions of the law.
+ After the deadline for public notification as specified in point b, paragraph 3 of Article 18 of Decree 25/2024/ND-CP, the State-level Council completes the application file and submits it to the President of the State-level Council. The Ministry of Health sends the file to the Ministry of Internal Affairs for evaluation and submission to the Prime Minister for consideration and decision by the President.
