Based on Article 19 of the Law on Health Insurance 2008, the replacement of health insurance cards in Vietnam is regulated as follows:
Exchanging health insurance cards
1. Health insurance cards shall be exchanged in the following cases:
a) Torn, damaged, or ruined cards;
b) Change in the initial medical examination and treatment registration site;
c) Incorrect information on the card.
2. Documents for exchanging health insurance cards include:
a) Application for card exchange by the health insurance participant;
b) Health insurance card.
3. Within 7 working days from the date of receipt of the full documents as prescribed in Clause 2 of this Article, the health insurance organization must exchange the card for the health insurance participant. During the waiting period for the exchange, the cardholder is still entitled to the benefits of a health insurance participant.
4. Persons who exchange health insurance cards due to torn, damaged, or ruined cards must pay a fee. The Minister of Finance stipulates the fee for exchanging health insurance cards.
Thus, students exchange their health insurance cards in the following cases:
- Torn, damaged, or ruined cards;
- Change in the initial medical examination and treatment registration site;
- Incorrect information on the card.

Based on Article 12 of Decree 146/2018/ND-CP, the student health insurance card issued by the social insurance agency presents the following information:
- Personal information of the health insurance participant, including: Full name; gender; date of birth; residential or workplace address.
- Health insurance benefit level as prescribed in Article 14 of this Decree.
- The validity period of the health insurance card.
- Initial health insurance medical examination and treatment registration site.
- Continuous participation period of 5 years or more for individuals who must co-pay for medical examination and treatment costs. The continuous participation period is recorded on the health insurance card following successive use periods; interruptions must not exceed 3 months.

1.3. Date of birth: printed as day, month, year.
1.4. Gender: printed as male or female according to the registration documents for health insurance participation.
1.5. Health insurance benefit level code: printed (1 character: in order from 1 to 5) indicating the benefit level of the health insurance participant.
