Section (1): Enter the name of the superior authority of the residence registration agency;

Section (2): Enter the name of the residence registration agency;

Section (3): Other confirmation contents (e.g., confirmation of previous permanent residence registration, temporary residence registration, etc.);

Section (4):

- Valid for 06 months from the date of issuance for cases where the place of residence of a person has neither permanent residence nor temporary residence due to insufficient conditions to register for permanent or temporary residence, the current place of residence is considered the person's current place of stay; if there is no specific place of stay, the current place of stay is determined as the commune-level administrative unit where the person is actually living. Persons without a permanent residence or temporary residence must report their residence information to the residence registration agency at their current place of stay.

- Valid for 30 days from the date of issuance for cases of residence confirmation.

- If there are changes or adjustments to a citizen's residence information that are updated in the residence database, this confirmation is invalid from the time of change.
