(1) Contents of military service health card
- Part I - Resume and medical history:
+ Curriculum vitae written by the Commune Military Command;
+ Medical history: recorded and responsible by the commune health station.
- Part II - Health examination: Recorded by the District Military Service Health Examination Council on the left half and the Military Unit Health Examination Council on the right half.
(2) Management of military service health cards
+ When not yet enlisted, the military service health card is managed by the District Military Command;
+ When a citizen enlists, the military service health card is given to the receiving unit and managed at the regimental military medical facility or equivalent;
+ Launching troops and handing over management to the District Military Command.
(3) Military service health certificate is only valid when:
+ According to the prescribed form;
+ Write with a pen or ballpoint pen, not with a pencil;
+ Writing is clear, no erasures, no abbreviations;
+ Completely write down the contents specified in the military service health card;
+ No more than 06 (six) months from the date the Military Service Medical Examination Council concludes on health status, except in cases of special health developments.

- Before the District Military Service Council issues an order calling citizens to enlist, the District Health Department must complete the list of healthy citizens according to regulations, complete the military service health card, and hand it over to the District Military Command.
- Based on the coordination plan between the receiving unit and the locality, the District Military Command hands over military service health cards of citizens meeting health standards to the military medical unit of the receiving unit.
- The military medical unit receiving troops organizes research in advance on the health records of citizens called to enlist. In cases of health suspicion, request the District Military Service Council to check and come to a conclusion.
- Summarize the results report according to the form in Appendix 5 issued with this Circular (localities assigning troops according to Forms 4a, 4b; military medical units receiving troops according to Form 4c).
- The receiving unit must organize a health check for new soldiers immediately after returning to the unit to classify and conclude health, detect cases of insufficient health to return to the locality, and be responsible for notifying the District Military Command:
+ Number of citizens meeting health standards;
+ Number of citizens who do not meet health standards and need to be returned to their localities (due to errors in the examination process where the disease has not been found or new diseases arising during the training of new soldiers).
- Regulations on returning citizens who do not meet health standards to localities
+ Citizens who do not meet the health standards are returned to their localities when the results of the health check are determined by the Chief of Military Medicine at the regimental level, equivalent, or higher and directly approved by their superiors.
+ Health records of citizens who do not meet health standards are returned to the locality. They must have a full military service health card from the local Military Service Health Examination Council with a section recording the examination results and health conclusions of the unit's Health Review Council.
+ Returning citizens who are not healthy enough to return to their localities shall comply with the regulations of the Ministry of National Defense. In case of necessity, the local health agency will organize a re-examination (within 7–10 days from the time the unit hands over the unhealthy citizen to the locality). In case of disagreement with the conclusion of the unit's Health Review Council, the District Military Service Council transfers the citizen's file to the Provincial Medical Examination Council to organize a health assessment.
