Procedures for granting certificates of relatives of meritorious persons in Vietnam (Internet image)
Applications for certification of relatives of meritorious persons include:
- An application form for certification of relatives of meritorious persons.
- Certified copy from one of the following documents:
+ A certificate of meritorious service or a medal for a person who has engaged in revolutionary activities, resistance war, defended the Fatherland or performed international obligations and was imprisoned or exiled by the enemy; Orders, Medals of Resistance; Medals, Victory Medals; Commemorating the "Fatherland's merits" medal; equal to "Being meritorious to the country".
+ Papers issued by a competent authority, confirming the relationship with the person with meritorious services, including: citizen identification.
Step 1: A person with meritorious services or a relative of a person with meritorious services shall make an application for a certificate of identification of relatives of a meritorious person and send it to the commune-level People's Committee of the place where he/she permanently resides, enclosed with a certified copy of one of the papers according to the provisions of Clause 1, Article 116 of Decree 131/2021/ND-CP (amended and supplemented in Clause 4, Article 13 of Decree 104/2022/ND-CP).
Step 2: The commune-level People's Committee of the place of permanent residence, within 05 working days from the date of receipt of complete papers, is responsible for issuing a certificate of relatives of the person with meritorious services according to Form No. 45, Appendix I of Decree 131/2021/ND-CP.
Applications for request for modification or supplementation of personal information in the profile of a person with meritorious services includes:
- An application form made according to Form No. 26, Appendix I to Decree 131/2021/ND-CP.
- Certified copies of the following documents:
+ For people with meritorious services: citizen identification or people's identity card.
+ For martyrs' relatives or beneficiaries of martyr's worship, relatives of deceased meritorious people: citizen identification or people's identity card.
Step 1: People with meritorious services; martyrs' relatives or beneficiaries of martyr's worship allowance; the relatives of the deceased person shall make an application using Form No. 26, Appendix I of Decree 131/2021/ND-CP, and send it to the Department of Labor, War Invalids, and Social Affairs, where the records of the person with meritorious services are managed, accompanied by certified copies of the papers specified at Point b, Clause 3, Article 130 of Decree 131/2021/ND-CP (amended and supplemented in Clause 4, Article 13 of Decree 104/2022/ND-CP).
Step 2: Department of Labor, War Invalids, and Social Affairs, within 12 days from the date of receipt of complete documents, is responsible for checking and comparing information and attaching documents prescribed at Point b, Clause 3, Article 130 of Decree 131/2021/ND-CP (amended and supplemented in Clause 4, Article 13 of Decree 104/2022/ND-CP), and copies of documents requested to be amended and supplemented with information shall be sent to competent agencies for settlement.
Step 3: Within 12 days from the date of receipt of a document amending and supplementing information from a competent agency, the Department of Labor, War Invalids, and Social Affairs is responsible for issuing a decision to amend or supplement information in the profile of the person having meritorious services according to Form No. 75, Appendix I of Decree 131/2021/ND-CP, and sending a notice of information adjustment to the Ministry of Labor, War Invalids, and Social Affairs.
