Procedures for issuance of Certificate of establishment registration of establishments providing domestic violence prevention and control support services guided in Decision 3657/QD-BVHTTDL 2023, effective from December 25, 2023, specifically as follows:
- Number of documents: 01 (set).
- Document components:
+ Application for the first issuance of a registration certificate to establish a facility providing domestic violence prevention and control support services according to Form No. 15 of the Appendix issued with Decree 76/2023/ND-C;
+ Draft operating regulations for facilities providing domestic violence prevention and control services;
+ Service price list (if any);
+ Profile of the leader:
(1) Health examination certificate from a competent medical facility according to the provisions of the law on health examination;
(2) Notarized copy of a university degree or higher in the field or training major related to the service registered to provide;
(3) Notarized or authenticated copies of diplomas, certificates, and other certifications related to establishment registration services;
(4) A commitment that you have not been prosecuted for criminal liability or have not been sanctioned for administrative violations in the field of domestic violence prevention and control according to Form No. 16 of the Appendix issued together with Decree 76/2023/ND-C;
+ Request records of employees directly involved in providing domestic violence prevention and control support services:
(1) Health examination certificate from a competent medical facility according to the provisions of the law on health examination;
(2) Notarized, authenticated copy of a high school diploma or higher;
(3) Notarized and authenticated copies of diplomas related to the service expected to be provided;
(4) Notarized or authenticated copy of the certificate of training in knowledge and skills on domestic violence prevention and control.
- How to submit application:
+ Submit 01 set of documents directly, by post, or electronically (via the National Public Service Portal) to the specialized agency on culture, sports, and tourism of the Provincial People's Committee (Department of Culture, Sports, and Tourism/Department of Culture and Sports of the province, city, or province) where the facility is located.
+ In case of electronic submission, the dossier components as sent directly are taken from the original or in digitally signed PDF format. The document name is set to correspond to the name of the attached paper or document.
- Organizations and individuals wishing to establish a facility submit 01 set of application documents for the first issuance of a Certificate of Registration to establish a facility providing domestic violence prevention and control services to the specialized agency on culture, sports, and tourism of the Provincial People's Committee (Department of Culture, Sports, and Tourism/Department of Culture and Sports of the province or city) where it is headquartered.
- In case the application is invalid, within 05 working days from the date of receipt, the receiving agency shall notify the applicant in writing to complete it;
- Within 10 working days from the date of receiving a valid application, the agency receiving the application shall issue the first certificate to the facility providing domestic violence prevention and control services according to Form No. 17 of the Appendix issued with Decree 76/2023/ND-CP. In the event of a refusal to issue a certificate, a written response must be provided clearly stating the reason.
