Instructions for filling out the application form:

Item (1): Enter the name of the enterprise requesting the issuance, extension, or re-issuance of the license.

Item (2): Write "issuance" or "extension" or "re-issuance" as requested by the enterprise.

Item (3): The competent state authority as stipulated in Article 15 of Decree 23/2021/ND-CP includes: Provincial People's Committee or Department of Labor - Invalids and Social Affairs authorized by the Provincial People's Committee where the enterprise is headquartered.

Item (4): Enterprise code according to the business registration certificate.

Item (5): Title of the legal representative of the enterprise engaging in labor service activities.

Item (6): The license code for labor service operations that has been issued (if any).

Item (7): Date of issuance of the license that has been issued (if any).

Item (8): State the reason under Clause 1, Article 20 of Decree 23 if it falls into one of the cases of re-issuance of the license as follows:

- Changing one of the contents of the issued license, including: enterprise name; headquarters address but still within the province where the license was issued; the legal representative of the enterprise;

- The license is lost;

- The license is damaged and lacks complete information;

- Changing the headquarters address to a different province from where the license was issued.

Item (9): Title of the legal representative of the enterprise.
