- Score 6: Indicates very poor health.

* How to record military service health card:

- For each specialty, after examining, the doctor will give a score in the column "Score"; in the column "Reason" must write a summary of the reason for that score; in the "Sign" column, the examining doctor must sign and clearly write his or her name;

- The conclusion, the Chairman of the Health Examination Council, based on the points given in each criterion to conclude and classify health according to regulations, writes in numbers and words (the parts in words are in parentheses). ;

- In case of doubt that the score cannot be given immediately, the Health Examination Council may send the citizen to be examined at a hospital for more accurate conclusions;

- In case no conclusion is reached, send the citizen to the nearest specialized hospital for examination and diagnosis. The maximum time is 7–10 days to reach a conclusion, and only do it in necessary cases;
