Application for pharmacy practice certificate in Vietnam 

Individuals wishing to apply for a pharmacy practice certificate need to prepare a set of application for pharmacy practice certificate , including the following documents:

- An application form for a pharmacy practice certificate.

- Certified true copy of the qualification.

If the qualification is issued overseas, its certified true copy must be enclosed with an equivalence certification issued by a competent authority

-  The original copy or certified true copy of the certificate of internship

If the internship took place in more than one facility, the internship duration will be the total duration of internship at the facilities according to the certificates of internship issued by such facilities;

- If the scope of practice covered by the pharmacy practice certificate applied for requires different internship durations and facilities, the application shall be enclosed with a certificate of internship of one or several facilities that meet requirements of each position.

In cases where the scope of professional activities has the same requirements for practice time and professional practice facilities, separate certification is not required for each scope of professional activities.

- The original copy or certified true copy of the confirmation of examination result if the pharmacy practice certificate has to be obtained after passing an examination;

Note: Documents issued by overseas competent authorities must be consularly legalized and enclosed with notarized Vietnamese translations.

Procedures for issuing the pharmacy practice certificate in Vietnam

The applicant for issuance, reissuance or adjustment of the pharmacy practice certificate shall submit an application directly or by post to:

- The Ministry of Health of Vietnam if the pharmacy practice certificate has to be obtained after passing an examination;

- Department of Health of the province if the pharmacy practice certificate may be granted without an examination.

After receiving the application, the receiving authority shall give the applicant the application form.

If the application is satisfactory, the receiving authority shall issue the pharmacy practice certificate within 15 days from the day on which the application is received; if the application is rejected, provide written explanation;

If the application is not satisfactory, the receiving authority shall request the applicant to complete the application within 10 days from the day on which the application for issuance of the pharmacy practice certificate is received

After receiving the amended and supplemented dossier, the dossier-receiving agency shall return to the requester the receipt of the amended and supplemented dossier according to and perform the following actions:

- In case the modified or supplemented dossier does not meet the requirements, the application-receiving agency shall notify the applicant in writing to continue to amend and supplement the dossier as prescribed.

- In the event there is no request for amendment or supplementation to the application for modification or supplementation, the application-receiving agency shall issue the pharmacy practice certificate as prescribed.

Within 06 months from the day on which additional documents are requested in writing by the receiving authority, the applicant for issuance, reissuance or adjustment of the pharmacy practice certificate shall submit additional documents as requested. If the applicant fails to satisfy such request by the aforementioned deadline or the application is not satisfactory within 12 months from the first time it is submitted, the application will be rejected.

Within 05 working days from the date of issuance, reissuance or adjustment of the pharmacy practice certificate, the receiving authority shall update the following information on its website:

- Full name, date of birth of the holder of the pharmacy practice certificate;

- Number of the pharmacy practice certificate;

- The scope of practice.

The pharmacy practice certificate will be made into 02 copies. One of them will be given to the applicant, the other retained by the issuing authority.
