Vietnam’s forms of reports on supervision and assessment of investment from September 1, 2023

- Forms of reports on overall supervision and assessment of investment for the whole year:

Form No. 01: Report on supervision and assessment of investment for the whole year.

- Forms of reports on supervision and assessment of public investment projects and projects with non-public investment state capital:

+ Form No. 02: Report on initial assessment;

+ Form No. 03: Report on periodic supervision and assessment during the investment stage (6 months or the whole year);

+ Form No. 04: Report on mid-term or stage-based assessment;  

+ Form No. 05: Report on supervision and assessment prior to adjustments to the investment project;

+ Form No. 06: Irregular assessment report;   

+ Form No. 07: Report on supervision and assessment of investment completion;  

+ Form No. 08: Report on periodic supervision and assessment during utilization and operation (6 months or the whole year);

+ Form No. 09: Report on impact assessment;

+ Form No. 10: Report on general supervision and assessment of investment projects under management in the year.

- Forms of reports on supervision and assessment of public-private partnership investment projects:

+ Form No. 11: Report on periodic supervision and assessment during the investment stage (6 months or the whole year);

+ Form No. 12: Report on periodic supervision and assessment during utilization and operation (6 months or the whole year).

+ Report on supervision and assessment prior to adjustments to the investment project shall comply with Form No. 05 prescribed in Point d Clause 2 of this Article;

+ Reports on mid-term or stage-based assessment, terminal assessment, and impact assessment shall comply with the corresponding forms prescribed in Clause 2 of this Article.  

- Forms of reports on supervision and assessment of investment projects with other funding sources:

+ Form No. 13: Report on periodic supervision and assessment during the investment stage (6 months or the whole year);

+ Form No. 14: Irregular assessment report;   

+ Form No. 15: Report on supervision and assessment prior to adjustments to the investment project;

+ Form No. 16: Terminal assessment report;   

+ Form No. 17: Report on periodic supervision and assessment during utilization and operation (6 months or the whole year);

+ Form No. 18: Report on impact assessment.

- Forms of community investment supervision reports:

Form No. 19: Report on community investment supervision.
